超声二维斑点追踪技术评价缩窄性心包炎患者左房功能

目的 应用超声斑点追踪技术(STE)评价缩窄性心包炎(CP)患者左房功能,探讨其临床应用价值.方法 对36例CP患者应用Simpsons法计算左房被动射血分数和主动射血分数,应用斑点追踪技术测量左房侧壁及房间隔的二维应变及应变率.并与40例正常人比较.结果 (1)CP患者左房最小容积、左房被动射血分数、左房主动射血分数较对照组显著减低(P<0.05);(2)CP患者左房侧壁及房间隔处心肌应变(S)、收缩期应变率(SRs)、舒张早期应变率(SRe)较对照组显著减低;左房侧壁舒张晚期应变率(SRa)显著减低(P<0.05),房间隔处SRa较对照组无差异.结论 CP患者左房功能减低,心包粘连主要影响左房泵功能.二维斑点追踪技术可以准确评价CP患者左房功能.

Evaluation of Left Atrial Function in Patients with Constrictive Pericarditis by Two-dimensional Speckle Tracking Echocardiography

Objective To evaluate the functions of the left atrium in patients with constrictive pericardium (CP)by two-dimensional speckle tracking echocardiography (STE) and its clinical value.Methods The studied cases consisted of 36 patients with CP and 40 healthy subjects.Left atrial passive ejection fraction (LAPEF) and left atrial active ejection fraction (LAAEF) were calculated using Simpson's method.The analyses were performed to get atrial longitudinal peak systolic strain (S),peak systolic stain rate (SRs),peak early diastolic stain rate (Sre) and peak late diastolic strain rate (Sra) of the interatrial septum and the lateral wall of the left atrium using speckle tracking echocardiography.Results LAPEF and LAAEF were significantly decreased compared with those in the control group (P<0.05).S,SRs,Sre of the interatrial septum and the lateral wall and Sra of the lateral wall were decreased in patients with CP,which were lower than those in the control group (P<0.05),but Sra of the interatrial septum didn't decrease compared with that in the control group (P＞0.05).Conclusions The left atrial functions have been decreased in patients with CP.Besides the pump function of left atrium was mainly related to the stiff pericardium.STE can accurately obtain parameters that provide unique data global and segmental atrial functions.
