脐带扭转结合胎心监护及脐血流参数与妊娠结局临床分析

摘    要:目的 探讨脐带扭转结合胎儿心电监护及脐动脉血流参数与妊娠结局的关系,寻找正确的处理方法,降低围产儿病率.方法 选取2004年6月~2006年6月在我院待产的孕妇发生脐带扭转,同时有胎儿心电监护及脐动脉血流检查的356例临床资料进行回顾性分析.结果 脐带扭转对胎儿造成的影响,如胎儿窘迫、胎儿宫内发育迟缓、胎死宫内、阴道手术产率、剖宫产率均高于脐带正常组.结论 脐带扭转与胎儿窘迫、预后及分娩方式密切相关.胎儿心电监护及脐动脉血流S/D值可提示胎儿缺氧和预后,有重要的临床价值.一经确诊应加强孕期监护,适时终止妊娠.

Abstract:Objective:Study the reaction of torsion of cord with pregnancy ending,combine the CTG and the blood flow paremeter of umbilical cord artery,to found the cure method in order to reduce the mortality rate.Methods :365 pregant women with torsion of cord,CTG and cord artry test in our hospital waiting for delivery from 6,2004 to 6,2006.Result :Torsion of cord can affect fetal,for example the rate of the fetal distress,FGR,fetal die,surgery rate on vagina,caesarean birth rate is higher than those which with normal cord.Conclusion :Torsion of cord has closely relation with fetal distress,prognosis and the way of delivery.Fetal CTG and blood flow of cord artery S/D can point out the lack of oxygen in fetal and prognosis,which has important clinical value.If diagnosed definitely,they should given intensive monitoring,select appropriate time to delivery.
