急性冠状动脉综合征患者MMP-9、MMP-2的临床意义

目的观察不同冠心病患者的血清的基质金属蛋白酶(MMPs)及其演变,探讨MMPs与急性冠状动脉综合征的关系。方法用酶联免疫吸附法(ELISA)检测了32例急性心肌梗死(AMI),30例不稳定型心绞痛,20例稳定型心绞痛和16例健康对照者血清MMP-9,MMP-2和组织型基质金属蛋白酶抑制物-2(TIMP-2)水平,并比较上述指标与不同类型冠心病的关系及其演变。结果AMI患者血清MMP-9水平高于不稳定型心绞痛组、稳定型心绞痛组和对照组,分别为(191.91±150.10)ng/ml,(110.84±96.29)ng/ml,(69.30±55.85)ng/ml和(80.13±34.94)ng/ml。MMP-2、TIMP-2四组间比较差异无显著性。MMP-9在AMI患者中发病早期升高后持续到3~5d。MMPs与心肌损伤标志物无相关关系。结论血清MMP-9是预示斑块破裂中的主要MMPs之一,但它不能预测心肌坏死的程度。

Value of serum MMP-9, MMP-2, TIMP-2 in acute coronary syndrome

Objective To investigate the difference of serum MMP-9,MMP-2,TIMP-2 levels in different subtype of coronary heart diseases and their variety and to explore the relation between MMPs and the acute coronary syndrome(ACS).Methods The levels of serum MMP-9,MMP-2,TIMP-2 among 32 acute myocardial infarction(AMI),30 unstable angina pectoris(UAP),20 stable angina pectoris(SAP) and 16 normal healthy control subjects were measured by enzyme-linked immunosorbent assay,and compared the evaluating values among different types of coronary heart disease and their variety.Results Serum MMP-9 levels were significantly higher in the AMI group than other groups. They were 191.91±150.10(ng/ml),110.84±96.29(ng/ml),69.30±55.85(ng/ml)和80.13±34.94(ng/ml).Serum MMP-2,TIMP-2 levels were not significant.Serum MMP9 level elevated and lasted for 3-5days in AMI group.MMPs levels were not correlated with cardiac enzymes.Conclusion MMP-9 is one of the most important MMPS in the rupture of the atherosclerotic plaque,but MMPs didn't predict the degree of the cardiac necrosis.
