滑脱钉棒系统在腰椎滑脱、不稳伴继发性椎管狭窄减压植骨内固定术中的应用

摘    要:目的 探讨腰椎滑脱、不稳伴继发性椎管狭窄的手术要点及滑脱钉棒系统在术中的作用.方法 总结2002 年8月-2011年7月采用滑脱钉棒系统治疗腰椎滑脱、不稳伴继发性椎管狭窄切开复位,减压植骨融合内固定治疗32 例患者的临床资料.结果 32 例随访6~24 月,平均12.5月.术后跛行改善者32 例(100%),下肢肌力障碍恢复22 例(68.75%),10 例肌萎缩均有不同程度恢复.16 例感觉障碍术后均有恢复,6 例完全恢复,10 例部分恢复.结论 在腰椎滑脱患者中,滑脱钉棒系统内固定植骨术对腰椎滑脱的患者可起到使滑脱椎体复位或部分复位,相邻椎体间稳定的作用,并可使摘除椎间盘后的相关间隙高度得到维持,减少残余或变性的组织向椎管内脱出.

Slipped Screw Rod System in Lumbar Spondylolisthesis,Instability with Secondary Spinal Stenosis Decompression Bone Grafting and Internal Fixation

Abstract:Objective :To study of lumbar spondylolisthesis, instability with secondary spinal stenosis operation points and slip screw rod system in the operation of the role and significance of. Methods : Summary of the 2002August - 2011July using slip screw rod system in the treatment of lumbar instability with secondary stenosis open reduction, decompression and bone graft fusion internal fixation in the treatment of 32cases of patients with clinical data. Results :32cases were followed up for 6- 24 months, mean 12.5months. Postoperative lameness improved in 32 cases (100%), muscle strength of lower limbs disorder recovery in 22 cases (68.75%), 10eases of muscle atrophy have different degrees of recovery. 16eases of sensory disturbance after operation were recovered, 6cases recovered completely, 10 cases recovered partially. Conclusion : In patients with lumbar spondylolisthesis with pediele screw fixation system, internal fixation and bone grafting surgery for lumbar spondylolisthesis patients can play the spondylolisthesis reduction or part reset, adjacent intervertebral stabilizing effect, and can make the removal of intervertebral disc associated clearance height is maintained, reduce the residual or degeneration of tissue into the spinal canal prolapse.
