宫腔镜下输卵管插管疏通治疗不孕161例分析

目的：探讨官腔镜下输卵管插管疏通术在治疗不孕的115床效果。方法：采用一根特制进口的输卵管导丝在官腔镜指引下找到输卵管开口，于直视下将输卵管导管丝缓缓插入疏通输卵管，然后拔出导丝注入美兰稀释液，如注入通畅无阻力且无返流，提示输卵管管腔已疏通。结果：161例均在输卵管输液器加压通水失败后以TERUMO导丝疏通，有9例一侧输卵管疏通时造成输卵管峡部穿孔，对侧成功，还有9例16条输卵管开口显示不清，在下次月经干净3d第二次疏通成功。手术成功率69％。结论：该术式操作简便，术野清晰，是集诊断与治疗于一体的微创手术，副作用少，痛苦小，具有可反复进行的优点，且在官腔镜下直视插管，可避免损伤内膜。官腔镜下输管疏通术，是一项有价值的可广泛应用于临床的技术。

The Analysis of 161 Cases of Infertility Treated with Hysteroscopic Tubal Cannulation and Hydrotubation

Objective： To evaluate clinical effect of hysteroscopic tubal cannulation technique and hydrotubation in treatment of infertility. Methods：With a special guide wire tube imports under the guidance found in hysteroscopic tubal opening, straight down the fallopian tube in the catheter slowly into the clear tube wire, then pull out the guide wire into the methylene blue dilution, such as into the smooth and without resistance without reflux, suggesting that tubal lumen is clear.Results：161 patients were in the fallopian tubes through the water pressurized infusion failed to TERUMO guide wire to clear, there are 9 cases the side of the fallopian tubes caused by tubal isthmus clear perforation, contralateral success, there are 9 cases of 16 tubal opening show is unclear, in the next three days the second time period a clean clear success. 69% success rate.Conclusion：The procedure is simple, with the clear operative field, it is a diagnosis and treatment in one of the minimally invasive surgery, less side effects and pains, it can be repeated with the advantages of intubation under direct vision and in hysteroscopy can be avoided intimal injury.Dredge pipeline and hysteroscopic surgery, is a technology which can be widely used and valuable in clinical practice.
