双侧脑室钻孔加椎管内置管引流及尿激酶冲洗治疗脑室出血的护理研究

目的探讨脑出血破入脑室患者早期行双侧脑室钻孔加椎管内持续置管引流及尿激酶灌注冲洗的护理。方法将112例脑出血破入脑室患者随机分为两组,实验组56例患者在出血6h内行双侧脑室钻孔引流术,术后第2天开始行腰穿术椎管内置管持续引流置换脑脊液,以及尿激酶脑室引流管灌注冲洗,加强监护,进行体位和引流管护理,严格无菌技术操作。与对照组56例患者做一样的术后治疗,每天要做一次腰穿,术后不置管对比。结果实验组治疗及时,腰穿次数减少4～6次,医护的工作量和患者的经济负担、并发症与对照组比均具有可比性,P<0.01。结论早期双侧脑室钻孔引流,腰穿加置管引流,加强护理,可减少患者并发症和经济负担,减少医护工作量。

Nursing Research of Bilateral Ventricle Boring Plus Intracathecal Space Drainage and Urokinase for Treatment of Intraventricular Hemorrhage

Objective To explore the nursing of early bilateral ventricle boring plus intracathecal space continual drainage and urokinase for treatment of intraventricular hemorrhage.Methods 112 cases of intraventricular hemorrhage patients were divided randomly into two groups.The patients(n=56)received bilateral ventricle boring drainage in six hours after hemorrhage in the experiment group;the continual drainage through lumbar subarachnoid space catheter and urokinase were performed from the second day after operation;strengthened the monitoring and nursing of patients,strict aseptic manipulation;the patients(n=56)in the control group received the same operation,but no drainage tube was inserted after lumbar puncture.The results of two groups were compared.Results The times of lumbar puncture,workload of medical staffs,economic burden of patients and complications in the experiment group were lower than those in the control group(P<0.01).Conclusion Early bilateral ventricle boring drainage,catheter drainage through lumbar puncture and strengthening nursing can decrease the complications and economic burden of patients,and reduce the workload of medical staffs.
