超声引导下右颈内静脉置管建立紧急血液透析路径

目的探讨超声引导下右颈内静脉置管建立紧急血液透析路径的可行性。方法在高频彩色多普勒超声实时引导及监测下,为38例急诊血液透析患者行右颈内静脉双腔导管置入术。结果38例患者中37例1次置管成功,1例因颈内静脉血流量不足和血管张力低而置管失败。本组患者置管时间13～738d,平均95d。结论高频彩色多普勒超声监测下能清晰分辨右颈内动静脉,可避免置管盲目穿刺造成的组织损伤,很好解决了急诊透析的血管通路,且根据病程需要可做中长期留置管。

Ultrasound-guided double channel catheter embedded in right jugular vein for emergent hemodialysis passage

Objective To set up emergent hemodialysis passage for acute renal failure patients.Methods Guided and monitored by high frequency color ultrasound,double channel catheter(Quinton,American) was embedded into right jugular vein.Results The catheter was inserted successfully only one procedure in 37 of 38 patients,Only one patient failed because of low venous pressure.Conclusions The carotid artery and jugular vein can be demonstrated clearly by high frequency ultrasound so that blind puncture can be avoided to reduce tissue injury.It may be a good,quick and easy method to set emergent hemodialysis passage.According to the severity of acute situation,the catheter may be remained middle or long term if necessary.
