摘    要:目的:探索低功率 C O2 激光焊接在尿道下裂修补中的可行性。方法:48 只家兔制成远端尿道下裂的模型,用 Mathieu 方法修补尿道下裂。对照组动物在放大镜下将皮瓣用70 无损伤缝线缝合到尿道,而实验组则使用低功率 C O2 激光焊接涂有蛋白制剂和萤光素的尿道及皮瓣对缘。结果:焊接组操作时间少,尿道腔内破裂压高,术后14 d 无尿瘘发生,对照组有2 例发生尿瘘;实验组未见明显的异物反应,炎症反应轻微,而对照组则有较强烈的异物及炎症反应。电镜检查显示激光焊接后吻合口处胶原胶联。结论:在家兔中使用低功率 C O2 激光辅以蛋白治疗尿道下裂有可能代替显微缝合。

Abstract:Objective:The purpose of the research was to test the feasibility of low power CO 2 laser welding in hypospadias.Metheds:A distal hypospadias was established in 48 rabbits and repaired by Mathieu method.In the control group ventrally placed skin flaps were sutured to the urethra and in the experimental group the skin flaps were welded to the urethra using a continuous wave CO 2 laser and human albumin with fluorescein.Results:The operative time and bursting pressure in experimental group was considerablly less than those in control group. No fistulas were seen in the welded urethra on day 14 after treatment and no significant foreign body reaction was identified in the experimental group but a strong foreign body reaction was found in 2 rabbits of control group. Conclusion:Hypospadias repair in the rabbit using low power CO 2 laser and albumin seems to be a feasible alternative to microsuture in rabbits.
