RetcamⅢ筛查婴幼儿眼底结果分析

摘    要:目的ReteamIII筛查婴幼儿眼底病变,将结果中早产儿视网膜病变(retinopathyofprematurity,ROP)的发病率及相关影响因素进行分析。方法选取从2012年5月至2013年4月间就诊于温州医科大学附属第二医院&育英儿童医院的婴幼儿共1097例,门诊筛查的婴幼儿333例(设为I组),新生儿科住院筛查的早产儿共764例(设为II组),将眼底筛查结果记录,并将ROP与胎数、出生体重、孕周、性别、新生儿呼吸窘迫综合征(neonatolrespiratorydistresssyndrome,NRDS)、新生儿窒息、新生儿高胆红素血症、颅内出血、先天性心脏病、动脉导管未闭(patentductusarteriosus,PDA)、新生儿贫血、新生儿败血症、支气管肺发育不良(bronehopulmonarydysplasia,BPD)、新生儿小肠结肠炎(neeroffzingenterocolitis,NEC)等疾病进行spearman相关性分析。结果1I组中ROP有98例,发病率为12.8%。胎数、出生体重、孕周与ROP显著相关(P〈0.05),ROP与NRDS、新生儿窒息、颅内出血、新生儿贫血、败血症、BPD及NEC呈显著相关(P〈0。05),与PDA、新生儿高胆红素血症、先天性心脏病无相关性(P〉0.05)。结论RetcamIII是检测婴幼儿眼底病变的重要手段,而ROP仍是早产儿最常见的眼病,温州地区ROP发病率为12.83%,胎数、出生体重、孕周、NRDS、新生儿窒息、颅内出血、新生儿贫血、败血症、BPD及NEC是ROP的重要影响因素。

Screening of infantsretina by RetcamIII

Abstract:Objective To investigate incidence and severity of retinopathy of prematurity (ROP) screened by ReteamlII and the related factors. Comparative case series were designed. Methods A study was performed among infants who required screening by RetcamlII in the second Affiliated Hospital & Yuy- ing Children's Hospital of Wenzhou Medical University, and carried out between May 2012 and April 2013. A total of 1097 infants met the study criteria, and were divided into group I ( n = 333 from outpatient screening) and group II ( n = 764 from inpatient screening). Variables were recorded, including birth weight (BW), gestational age (GA), number of births (NOB, singleton/twins/triplets), gender, neonatal respiratory distress syndrome (NRDS), patent ductus arteriosus (PDA), neonatal asphyxia, neonatal hy- perbilirubinemia, intracranial hemorrhage, congenital heart disease, neonatal anemia, neonatal sepsis, ne- crotizing enterocolitis ( NEC), and bronchopulmonary dysplasia (BPD). The results of ophthalmological ex- aminations by RetcamIII were also recorded. Spearman correlation analysis was used to identify the variables most significantly associated with ROP. Results There were 98 infants of ROP in group II (total 764, 12. 8% ). There was a significant association of ROP with variables ( P 〈0. 05), including BW, GA, and NOB. There was a significant association of ROP with diseases ( P 〈 0. 05), including NRDS, neonatal as- phyxia, intracranial hemorrhage, neonatal anemia, neonatal sepsis, NEC, and BPD. However, there was no association of ROP with diseases ( P 〉 0. 05 ), including PDA, neonatal hyperbilirubinemia, and con- genital heart disease. Conclusions RetcamIII can be used to screen retina of infants. ROP is still the mostcommon eye disease in preterm infants. The incidence of ROP was 12.83% in Wenzhou. Variables, inclu- ding BW, GA, NOB, NRDS, neonatal asphyxia, intracranial hemorrhage, neonatal anemia, neonatal sep- sis, NEC, and BPD, were the most significant factors associated with ROP.
