磁共振多技术联合应用在乳腺良恶性病变中的临床应用价值

摘    要:目的:探讨MRI多项技术在乳腺良、恶性病变诊断中的临床应用价值。方法:对39例乳腺疾病患者行T1WI、抑脂T2wI、动态增强(DCE—MRI)、弥散加权成像(DWI)扫描。使用后处理软件测量表观弥散系数(ADC值)、绘制时间一信号强度曲线。评价上述方法对乳腺疾病的定性诊断价值。结果:39例患者中,共43个病灶,恶性30例,良性13例。常规MR成像、动态增强成像诊断乳腺病变的敏感性分剐为73.3%(22/30),86.7%(26/30),特异性为61.5%(8/13),69.2%(9/13),准确率为69.8%(30/43),81.4%(35/43)。ADC值诊断及联合诊断的敏感性为分别93.3%(28/30),83.3%(25/30),特异性为76.9%(10/13),84.6%(11/13),准确性为88.4%(38/43)83.7%(36/43)。结论:乳腺磁共振多项技术联合应用,可提高病变诊断的特异性,对乳腺良、恶性病变的诊断有重要临床价值,并且为临床制定治疗、手术方案提供重要的依据。

Clinical evaluation of Breast Benign and Mafignant Lesions Diagnosis with MRI Multiple Technology Analysis

Abstract:Objective: To evaluate the clinical value of MRI multiple technology analysis diagnosis of breast benign and malignant lesions. Methods:39 cases patient of breast disease all received double -breast axial T1 WI,T2WI series axial fat - pressure series and ax- ial diffusion weighted imaging(DWI) scan ,dynamic contrast -enhanced scan. Using postl processing software to obtain the measurement of Apparent Diffusion Coefficient( ADC value) ,time signal strength curves. The validities of all the above methods were evaluated. Results: The sensitivity, speetivity, and accuracy of MRI rout - scanning and dynamic contrast - enhanced imaging respectively were73.3% (22/ 30 ) ,86.7% (26/30) ,61.5% ( 8/13 ) and 69.2% (9/13) ,69.8 ( 30/43 ) , 81.4% ( 35/43 ) respectively. The senificity, spectivity, and accuracy of ADC for were 93.3% (28/30) ,76.9% ( 10/13 ) ,88.4% (38/43)respectively. Tile senificity, spe -etivity, and accuracy of combination of dynamic contrast - enhanced MRI and DWI - ADC were 83.3% ( 25/30 ) , 84.6 ( 11/13 ) and83.7 % ( 36/43 ) respectively. Conclusion:The combination of MRI multiple technology is useful to increase specificity, all of the inspection method has important value in the diagnosis od breast benign and malignant lesions.
