掌腱膜部分切除术治疗掌腱膜挛缩症

目的 探讨掌腹腱膜挛缩症治疗的临床经验。方法 1980年以来，我院共治疗掌腱膜挛缩症22例，全部采用掌腱膜部分切除术。结果 19例病人获得满意的治疗效果，应用Tubiana’s法评价，手术后优良率为86．4％，效果良好。结论 本手术成功的关键在于：仔细分离皮瓣，避免皮肤坏死；避免血管神经束损伤；彻底切除挛缩腹膜，避免术后复发；彻底止血，防止血肿形成和感染；术后早期进行功能锻炼。

Treatment of Dupuytren's contracture with partial resection of palmar fascia

Objective To investigate the clinical experienc es for treatment of Dupuytren's contracture.Methods Twen-ty-two cases with Dupu ytren's contracture had been treated in our unit since1980.All cases were t reated with the same operation-partial resection of palmar fascia.Results Nint een cases showed comparatively satisfactory result.Tubiana's method of evaluati on showed the extension-flexion range of movement to be satisfactory by86.4 %.All over the outcomes were better.Conclusion We think the success of operat ion mainly depends on following key points,such as:(1)the flaps should be r aised carefully to avoid necrosis;(2)Avoid injury of neuro-vascular bundle ;(3)the pathological palmar fascia should be removed completely to avoid rec urrence;(4)hemostasis should be complete to avoid hematoma formation and pos toperative infection;(5)rehabilitation should be started earlier.
