2型糖尿病患者下肢动脉硬化的超声研究

目的应用超声观察2型糖尿病（2DM）患者外周动脉病变的影响，并探讨糖尿病周围血管病变的发生、发展规律，评价2DM的外周小血管病变。方法观察25例2DM患者，除外合并高血压病、急性冠脉综合症、陈旧性心肌梗塞、脑梗塞、心功能不全者；以健康成人25例为正常对照组。采用高频超声探头（频率3—11MHz）测量两组足背动脉内径、收缩期峰值血流速度（S）、舒张末期流速（D）、血管阻力指数（RI）、脉动指数（PI）等指标，同时观察血脂、血糖的变化。结果观察组TG、TC、LDL—C明显高于对照组，HDL—C明显低于对照组；足背动脉的内径、S、D、PI低于对照组，RI高于对照组；差异均有显著性。结论2DM其血脂水平及血管超声指数均与健康对照组有明显的改变，说明2DM存在脂代谢紊乱和下肢血管病变，超声能够准确反映其病变严重程度及血管功能。

Study on Remodeling of Atherosclerosis of Lower Extremity in Patients of Type 2 Dabetes Mellitus with High-Frequency Ultrasound

Objective To investigate.the Study on remodeling of atherosclerosis of periphery artery in patients of type 2 diabetes mellitus （2DM）with high -frequency ultrasound, discuss the law of the occurrence, development and degrade of diabetic peripheral angiopathy. Methods 25 2DM patients were in observe group, affiliatehy pertensire disease ,ACS, OMI, brain infarction, HF were excluded, and 25 health examiner were in control group. The cases in two group were all checked Dorsalis pedis artery with high -frequency ultrasound,the intima artery diameter （ A D ）, systolic velocity （ S）, diastolic velocity （ D ）, resistance index （RI） and pulsatility index （ PI ） were measured with CDFI. Blood glucose,lipid profile were also compared between the two groups. Results the TG, TC, LDL - C and RI in observe group is significantly higher than control group, HDL- C ,PI,S,D and AD are significantly lower （ P 〈 0.05, P 〈 0.05, P 〈 0.05, P 〈 0.05, P 〈 0.05, P 〈 0.05, P 〈 0.05, P 〈 0.05, P 〈 0.05 ）. Conclusion the lipid profile and vascular ultrasound index in patients of type 2 diabetes mellitus were very modification compared to health examiner group, It demonstrated that Lipid Metabolism disorder and lower extremity vascular lesion occurrenced in 2DM, high -frequency ultrasound can check the pathological changes and artery functions well and truly, It can provide a safely and effectively criterion on forepart diagnosis in coefficient of coincidence in Arteriosclerosis with 2DM.
