| 摘 要： | 目的 :探讨腹腔镜胆囊切除 (LC)术后二次黄疸的原因 ,以及治疗与预防的方法。方法 :分析 2例LC术后二次黄疸的临床资料。结果 :2例中第一次黄疸均为迷走胆漏引起 ,置管引流后黄疸消退 ;第二次黄疸为炎性瘢痕包裹压迫胆总管致胆总管闭塞引起 ,行胆肠吻合治愈。结论 :LC术后胆管损伤包括直接损伤和间接损伤 ,直接损伤可引起早期黄疸 ,间接损伤可引起远期黄疸。间接损伤所致外科黄疸临床罕见 ,应予重视。

| Abstract: | Objective:To investigate the reason,treatment and prevention of the second jaundice following laparo-scopic cholecystectomy(LC).Methods:The clinical data of2cases of the second jaundice following LC were analyzed and summarized.Results:In the2cases,the first jaundice was caused by aberrant biliary fistula and vanished after drainage treatment;the second jaundice was caused by the obliteration of the common bile duct because of the inflammatory scar,and was cured after cholangio-jejunostomy.Conclusions:The injury of the biliary duct following LC includes direct and indirect injury.The direct injury can lead to janudice in the earlier period.The indirect injury can lead to jaundice hap-pened in the late period,which is rare in clinical work,and should be paid more attention to. |
