高频CDFI超声对乳腺肿块诊断与病理对照分析

目的 探讨良恶性乳腺肿块高频二维声像图和CDFI血流状况与病理对照研究。方法58例乳腺肿块经高频超声观察其形态、边界、边缘、内部回声、有无后方衰减和侧方声影等，再用CDFI检测肿块内部与周边血流。结果在乳腺肿块声像图诊断中，以边界回声特征最为重要，它是肿块良恶性鉴别的关键。本组典型乳腺癌形态不规则，边界不清，内部回声不均，部分有钙化斑点，后方回声衰减，前缘与侧缘有强回声晕，同时CDFI检出丰富血流信号。不典型乳腺癌肿块边缘规则，后方回声无衰减，可见侧方声影。结论高频CDFI超声诊断乳腺肿块良恶性病变，符合率高，具有重要的临床价值。但要对不典型乳腺癌与乳腺良性疾病做出明确鉴别，尚待进一步研究。

High Frequency Color Doppler Flow Imaging in Diagnosis and Differential Diagnosis of Breast Masses: Compared with Pathology

Objective To explore the effects of high frequency CDFI in diagnosing and differentiating breast lumps compared with pathology. Methods A total of 58 cases with breast lumps were examined with high frequency ultrasound on which the shape, edge, inner echoes, rear attenuation and lateral shadowing, inner and peripheral blood flows were emphasized. Results Rim echoes were the key characteristics to differentiate benign and malignant masses. The of most malignant lumps in our cases had irregular shape, edge and irregular inside echoes with plentiful blood flows. Some cases had ealeifie spots in inner eehoos. Irregular echo enhancements in anterior and lateral borders were also shown in most mammary cancers. Some cases of our study with mammary cancers showed atypical presentations including malignant lumps with regular edge and lateral shadow without posterior attenuation. These changes might be associated with infiltrated growth of malignant cells and hyperplasia of. fiberous tissues. However, in benign tumors, smooth edges and lateral echo attenuation were demonstrated because of expansible growth in an intact membranee. Conclusions High frequency CDFI is of great value in diagnosing and differentiating benign and malignant breast turnouts. However, Exactly differential diagnosis of benign and malignant breast tumors needs further investigatation.
