一期前段玻璃体切割对儿童后发性白内障的影响

目的探讨一期前段玻璃体切割术对儿童后发性白内障发生的影响。方法 56例106眼儿童白内障患儿行白内障吸出及后囊膜环形撕除,按是否一期联合前段玻璃体切割分为两组,Ⅰ组为非联合组47眼,Ⅱ组为联合组59眼,2岁以上儿童一期行囊袋内植入人工晶状体,术后随访3～36个月。观察2组患儿术后视力,后发性白内障及其他并发症。结果两组后发障的发生率分别为：Ⅰ组14眼（29.8%）,Ⅱ组5眼（8.5%）,组间差异具有统计学意义（P〈0.05）。Ⅱ组中一期行囊袋内植入人工晶状体19眼中后发障的发生率为5.55%（1眼）,明显低于同期囊袋内植入人工晶状体40眼中患儿后发障的发生率17.5%（7眼）,差异具有统计学意义（P〈0.05）。结论一期前段玻璃体切割术能够减少或延迟后发性白内障的发生,同期行囊袋内植入人工晶状体效果更显著。

The effect of anterior ritrectomy in decreasing the incidence of capsule opacification in children

Objective To investigate the effect of anterior ritrectomy in decreasing the incidence of posterior capsule opacification （PCO） after cataract surrery in children. Methods Fifty-six pediatric cataract patients （106 eyes） aged from 0.25 years old to 12 years old received the cataract surgery,we reassigned to two groups,no-anterior vitrectomy group （25 cases,47eyes） and anterior vitrectomy group （31cases,59 eyes） . All eyes had posterior continuous curvilinear capsulorhexis,the acrysof intraocular lens （IOL） were implanted into children older than two years. The visual acuity , incidence of posterior capsular opacification and other complications were recorded and followed up from 3 months to 36 months. Results The after-cataract appeared 5 eyes （8.5%） in vitrectomy group and 14 eyes （29.8%） in novitrectomy group,there was statistical difference （P0.05） . Children aged 2 years or older with lens implantation in vitrectomy group,have less incidence of PCO （5.55% ,one of nineteen eye） than others （17.5% ,seven of fouty eyes）,there was statistical difference （P0.05） . Conclusion The anterior vitrectomy can decrease or prevent the occurrence of aftercataract obviously , especially to children with lens implantation at the same time.
