护理干预联合帕罗西汀治疗骨科慢性疼痛

目的探讨护理干预联合帕罗西汀对骨科慢性疼痛患者治疗的效果。方法将80例骨科慢性疼痛性疾病伴发抑郁的患者随机分为治疗组和对照组,每组40例。治疗组给在护理干预的同时予帕罗西汀20 mg/d,对照组给予护理干预和安慰剂1片/d。分别于治疗第4周末评价临床总体疗效。结果骨科慢性疼痛患者抑郁发生率56.3%（45/80）,严重抑郁发生率11.3%（9/80）;抑郁与疼痛病程和评分呈正相关（P〈0.05）;能明显提高治疗疼痛疗效,总有效率达90.0%,缓解抑郁症状。结论骨科慢性疼痛抑郁发生率较高,护理人员应高度重视评估患者疼痛状况及心理状况,把减轻疼痛和减少抑郁发生率作为护理目标,联合抗抑郁药进行治疗则可显著改善患者症状,甚至可完全消除疼痛。

Observation on therapeutic effects of nursing intervention with paroxetine on orthopedic chronic pain

Objective To investigate therapeutic effects of nursing intervention with paroxetine on orthopedic chronic pain.Methods 80 cases of orthopedic chronic pain patients with depression were selected and divided randomly into treatment group（n=40） and control group（n=40）.Treatment group was given treatment of nursing intervention with paroxetine,which were administered orally 20mg/d.Control group was given treatment of nursing intervention with placebo.Overall clinical effects of two ways were evaluated after 4 weeks of treatment.Results 56.3%（45/80） of patients with orthopedic chronic pain were depression and 11.3%（9/80）of them were major depression.Degree and score of pain were positively correlated with depression（P0.05）.Nursing intervention with paroxetine can improve therapeutic effects significantly in treatment of orthopedic chronic pain.The total effective rate was up to 90.0%.Conclusions There were high depression incidence rate in patients with orthopedic chronic pain.Nurses should pay attention to access the pain and psychological status of the patients to relieve the pain and depression effectively.The way of nursing intervention with antidepressant treatment not only improves symptoms,but also relieves the pain significantly.
