子宫肌瘤高强度聚焦超声治疗术后二维超声表现

摘    要:目的 探讨评价高强度聚焦超声(HIFU)治疗子宫肌瘤疗效的超声指标.方法 分析HIFU术后子宫超声图像特点,利用灰度处理软件对图像进行数字化处理.对比术前、术后子宫及肌瘤的体积、缩小率、肌瘤灰度值,以及内部均匀度的变化,寻找可用于评价HIFU疗效的客观指标.结果 HIFU治疗前后,子宫及肌瘤的体积均有较大变化(P〈0.05),肌瘤灰度值变化除术后6个月与术前无差异外,余差异均有统计学意义(P〈0.05),肌瘤内部均匀度的变化在术后1、3个月与术前比较差异无统计学意义(P〉0.05).结论 超声是评价HIFU治疗子宫肌瘤疗效最方便,具有较高临床应用价值的影像学检查方法,其声像图反映的子宫及肌瘤体积缩小、肌瘤内部灰度值的变化可以作为疗效评价的指标.

Abstract:Objective To explore ultrasonic evaluation index of uterus myoma after HIFU treatment. Methods The ultrasonic image characteristics of uterus after UIFU treatment were analyzed, and the sonograms was digitally processed by gray - scale image processing software. The volume of uterus and myoma, the reduction rate, gray value and internal uniformity of the myoma were compared before and after operation, and the objective indicators for evaluating the efficacy were searched. Results There was significant difference of the volume of uterus and myoma before and after operation ( P 〈 0. 05 ) , while there was no significant difference of gray value of myoma after operation, except before and 6 months after operation( P 〈 0.05 ) , and no significant difference of internal uniformity of myoma was observed before and after i, 3 month ( P 〉 0.05 ). Conclusion Ultrasound is a imaging method with high clinical value to evaluate HIFU treatment, which can reflect the size of uterus and myoma, gray - scale changes. They can be used as an indicator for curative effect evaluation.
