影像组学诊断胶质母细胞瘤的Meta分析

摘    要:目的系统评价影像组学方法对胶质母细胞瘤的诊断价值。方法计算机检索PubMed、EMbase、Web of Science和The Cochrane Library数据库,查找利用影像组学方法对脑胶质瘤进行分级或对脑胶质母细胞瘤和其他肿瘤进行鉴别诊断的相关研究,检索时限均为建库至2021年5月30日。由2名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险和研究质量后,使用Meta-Disc 1.4软件和RevMan 5.3软件进行Meta分析。结果共纳入37个研究,包括4748例患者。Meta分析结果显示:影像组学诊断胶质母细胞瘤的合并敏感度、特异度、诊断比值比和SROC曲线下面积分别为0.9195%CI(0.89,0.92)]、0.8895%CI(0.87,0.90)]、78.0095%CI(50.81,119.72)]和0.95,正确诊断胶质母细胞瘤的关键影像组学特征是病灶区域的强度特征和纹理特征。结论当前证据显示,影像组学对胶质母细胞瘤的诊断具有较高准确性。受纳入研究数量和质量的限制,上述结论尚待更多高质量研究予以验证。

Diagnostic value of radiomics in glioblastoma:a meta-analysis

Abstract:Objective To systematically review the value of radiomics in the diagnosis of glioblastoma.Methods PubMed,EMbase,Web of Science and The Cochrane Library databases were electronically searched to collect studies on radiomics in the grading of gliomas or the differentiation diagnosis from inception to May 30;,2021.Two reviewers independently screened literature,extracted data,and assessed the risk of bias and the quality of the included studies.Meta-analysis was then performed using Meta-Disc 1.4 software and RevMan 5.3 software.Results A total of 37 studies involving 2746 subjects were included.The results of meta-analysis showed that the pooled sensitivity,specificity,and diagnostic odds ratio for the diagnosis of glioblastoma by radiomics were 0.91(95%CI 0.89 to 0.92),0.88(95%CI 0.87 to 0.90),and 78.00(95%CI 50.81 to 119.72),respectively.The area under the summary receiver operating characteristic(SROC)curve was 0.95.The key radiomic features for correct diagnosis of glioblastoma included intensity features and texture features of the lesions.Conclusion The current evidence shows that radiomics provides good diagnostic accuracy for glioblastoma.Due to the limited quality and quantity of the included studies,more high-quality studies are required to verify the above conclusions.
