碘缺乏地区不同层次人群TSH水平的测定及其应用

摘    要:应用免疫放射测定法(IRMA)作纸片血斑促甲状腺素(TSH)测定,对不同层次人群的TSH水平进行测定。结果:(1)胎儿及新生儿组TSH值<5mIU/L者分别占68.4%和61.1%。其他各组(<1岁婴儿、7~14岁学生、孕妇A、孕妇B、育龄妇女和成人)TSH值<5mIU/L者占96.7%~100%,分别与胎儿及新生儿组比较,差异有极显著性(P<0.01)。(2)7~14岁学生、孕妇B、成人及育龄妇女的尿碘值≥100μg/L者分别为56.1%,53.2%,58.9%和52.2%。(3)居民盐碘含量<20ppm者占97.6%,提示该地区存在碘缺乏,以致胎儿及新生儿组TSH值向高值偏移,新生儿为监测碘缺乏唯一理想的人群。

Abstract:Measuring the TSH levels of blood spotted on the filter paper by immunoradiometric assay (IRM A), we compared the TSH values among different segments of the population in iodine deficiency areas. The results showed: 1)The percentages of TSH values less than 5 mIU/L in the fetus and newborn groups were 68.4% and 61.1% respectively. The percentages of TSH values less than 5 mIU / L among the other groups (< 1-year infants, 7-14-years students, pregnant women A, pregnant women B, women of reproductive age, and adults) were 96.7-100%, which showed significant statistical defferences (P<0.01) when compared with those of TSH values less than 5 mIU / L in the fetus and newborn groups. 2) The percentages of urinary iodine values longer than or equal to 100 ug/L in the four groups (7-14-years student, women B, women of reproductive age and adults) were 56.1%, 53.2%, 58.9% and 52.2% respectively. 3) 97.6% of the residents had salt intake less than 20 ppm iodine. The results indicate that there is still iodine deficiency in this area and the TSH values drift toward high levels in the fetus and newborn groups. Newborns are the ideal population for monitoring the iodine deficiency.
