病变组织部分切除皮瓣修薄术治疗肢体中期淋巴水肿

目的：研究和探索中期肢体淋巴水肿手术治疗的有效方法。方法：采用病变组织部分切除皮瓣修薄术治疗中期肢体淋巴水肿。结果：采用本术式治疗的50例患者中,有44例皮瓣成活较好,切口一期愈合,外形功能恢复比较理想;6例患肢皮瓣部分感染坏死、切口裂开,其中4例经切口清创换药,再次切除坏死皮肤创缘拉拢减张缝合后切口愈合,2例残余创面经换药植皮修复。所有患者术后患肢外形较术前明显变细,功能恢复良好。结论：对中期淋巴水肿患者,采用病变组织部分切除皮瓣修薄术是较好的治疗方法。

Excision of the lesion with thinning of skin flap for treatment of mid-stage lymphedema

Objective To study and search for the effective methods of operative treatment on mid-stage extremity lymphedema.Methods patients with mid-stage extremity lymphedema undertook surgical treatment by excision of the lesion with thinning of skin flap.Results In 50 patients undertaking this operation,44 cases acquired fair well recovery of their extremity appearance and functions,with well survival of skin flap and primary healing incision.In 6 patients the skin flap infected and necrosed partially,leading to the disruption of the wound.In 4 cases of the 6 patients,the wound healed through debridement and change of dressing and secondary resection of the necrosed skin and relaxation suture.In the other 2 patients,the remnant wound healed through change of dressing and skin replanting.In all patients,the affected extremity appearance became significantly thinner after operation than that before operation,with well-recovered function.Conclusion The excision of lesion with thinning of skin flap is a better treatment to those patients with mid-stage lymphedema.
