探讨呼吸暂停综合征对血压的影响

目的认识打鼾患者睡眠时血压的改变，初步探讨呼吸暂停综合征（OSAS）对血压的影响。方法对100例打鼾者进行Autoset诊疗系统监测及睡前、醒后肘部血压测定，将62例诊断为OSAS患者作为OSAS组，38例未达到诊断标准者作为对照组，并对两组进行动脉血氧饱和度监测，对两组进行上述各项指标的统计学处理，同时对OSAS伴高血压患者治疗1个月左右，重复上述测定。结果OSAS组睡前血压较晨醒后明显升高（P〈0．01），与对照组睡前血压及醒后血压比较呈显著性升高（P〈0．01），治疗组Sa02较对照组显著降低（P〈0．01），OSAS组有33例（53％）确诊为高血压病，经鼻面罩持续正压通气（nCPAP）治疗1个月，有19例患者血压明显下降，其中3例患者血压恢复正常。结论OSAS患者的低氧血症可能是相当部分OSAS伴发高血压的发病原因，nCPAP治疗有助于此类患者血压的恢复。

Effect on Blood Pressure of OSAS

Objective To observe the blood pressure changes during snoring and discuss the Effect on blood pressure of OSAS.Methods 100 cases of such patient were monitored the elbow blood pressure before and after sleep with Autoset system.62 cases were divided into OSAS group and the other were as control group.The data of two groups were compared and the results 1 month later after treatment were compared too.Results The blood pressure before sleep was higher than that in next morning in OSAS group and that was higher than that in control group with P<0.01.The SaO2 in therapy group was lower than that in control group.33 cases who were final diagnosis hypertension in therapy group were underwent nCPAP Bp of 19 cases among them decreased and 3 cases recovered.Conclusion The hypoxemia might be the etiologic reason of OSAS companied hypertension.The nCPAP might be benefit for recovery of Bp.
