子宫平滑肌瘤栓塞治疗的临床观察

目的:评价经导管子宫动脉栓塞(UAE)对子宫肌瘤的治疗效果。方法:回顾分析了2002年7月~2006年3月38例资料完整、诊断明确、有随访记录的子宫肌瘤行UAE的病例。主要症状包括经量增多、尿频、下腹坠胀疼痛及贫血等。其中双侧子宫动脉栓塞23例,单侧15例,栓塞剂采用PVA加明胶海绵颗粒。结果:38例UAE后临床症状明显改善;经量增多下腹部坠胀消失。栓塞后3个月复查B超,瘤体体积缩小38%~84.6%。结论:UAE治疗子宫肌瘤具有微创性、方便灵活性及适应证宽、疗效好等特点,将成为子宫肌瘤治疗新方法之一。

Evaluation of treatment of uterine leiomyoma using transcatheter uterine arterial embolization

Objective:To evaluate the effect of uterine arterial embolization(UAE)in the management of uterine leiomyoma.Methods:Thirty eight patients with uterine leiomyoma undergoing UAE from June 1998 to March 2003 were analyzed retrospectively,whose clinical symptoms were refractory vaginal bleeding,pelvic compression and pain,anemia,etc.Bilateral arterial embolization was performed in 23 cases,unilateral arterial embolization in 15 cases.The embolic materials were polyvinyl alcohol(PVA)and gelatin sponge particles.Results:Three months after the treatment,the size of the uterine leiomyoma decreased markedly(38%～84.6%)according to the sonographical examination,the clinical symptoms relieved markedly;there was no severe complication.Conclusion:Treating uterine leiomyoma through UAE is effective,mininvasive,safe and convenient,has extensive indication.UAE may be a promising new method for the treatment of uterine leiomyoma.
