探讨降钙素原极度增高患者心脏功能和容量负荷的临床价值

摘    要:目的探讨降钙素原(PCT)极度增高患者的心脏功能和容量负荷的临床应用价值。方法将7例抗感染治疗无效的血液PCT增高患者纳入研究,全部患者均给予超声心动图检查、病原学检测及评估心脏容量负荷指标血液脑钠肽(BNP)检测。同时给予限制输液速度、利尿等改善心功能的诊断性治疗措施,并对各项临床指标进行分析。结果全部患者均无严重感染的病原学证据;血液脑钠肽(BNP)均明显增高(989.6~5000pg/m1);超声心动图均存在不同程度的心脏功能不全和容量负荷过重;当患者容量负荷减轻时,BNP降低,PCT也随之下降。结论对一个PCT极度增高的患者,切不可只想到细菌感染和液体复苏,有必要了解和关注心脏功能和容量负荷,PCT明显增高的原因也可能是心功能不全。

Clinical value of learning extremely elevated procalcitonin in patients with heart function and volume load

Abstract:Objective To explore the clinical value of Learning extremely elevated procalcitonin (PCT) in patients with heart function and volume load. Methods 7 cases of anti-infective therapy in patients with elevated blood PCT were included in the study. All the patients were given echocardiography, pathogen detection and assessment of cardiac volume load index of blood-brain natriuretic peptide (BNP) testing. And also given the infusion speed limit, diuretic improve heart function diagnostic therapeutic measures. On various clinical parameters were analyzed. Results All patients had no evidence of serious infections etiology~ blood-brain natriuretic peptide (BNP) were significantly increased to 989.6 5000pg/ml~ eehocardiography there are different degrees of cardiac dysfunction and volume overload~ patient capacity reduce the load time, BNP decreased, PCT also decreased. Conclusion The extremely elevated PCT patients must think of bacterial infections and fluid resuscitation. It is necessary to understand and concerns of cardiac function and volume o- verload.
