子痫前期重度肾功能损害的相关因素及对母儿预后的影响

目的：探讨重度子痫前期肾功能损害的相关因素及对母儿预后的影响。方法回顾性分析2008年6月-2011年6月在新疆医科大学第一附属医院产科住院的孕28 w以上的354例重度子痫前期患者，将重度子痫前期患者按有无肾功能损害分为肾功能损害组及无肾功能损害组。分析比较两组患者的一般资料、实验室检查及母亲、胎儿与新生儿预后情况。结果子痫前期重度肾功能损害组与无肾功能损害组相比，收缩压、发病孕周及定期产检差异有统计学意义（P ＜0．05）。两组孕妇血清白蛋白、PT活动度、国际标准化比率、纤维蛋白原比较差异有统计学意义（P ＜0．05），在两组孕妇严重并发症的比较中，胎盘早剥、心衰、HELLP综合征发生率比较差异有统计学意义（P ＜0．05），在胎儿及新生儿预后的比较中，胎儿窘迫、早产、胎儿生长受限、死胎差异有统计学意义（P ＜0．05）。结论重度子痫前期肾功能损害的相关因素为收缩压、发病孕周、定期产检、血清白蛋白及凝血功能的相关检查，重度子痫前期肾功能损害与母儿的多种严重并发症有关。

Analysis of relative factors of severe Pre-eclampsia kidney function inj ury and the prognosis of pregnant woman,fetus and newborn

Objective To investigate the relative factors of severe Pre-eclampsia kidney function injury and the prognosis of pregnant woman,fetus and newborn.Methods Clinical data of 354 cases of pregnant women suffering from severe Pre-eclampsia whose gestational age were over 28 weeks in the First Affilia-ted Hospital of Xinjiang Medical University from June 2008 to June 2011.Patients were divided into two groups as the kidney function inj ury and the control group of kidney function normal.Results Systolic blood pressure,incidence of gestational age,regular obstetric examination,serum album,PT activity,In-ternational normalized ratio and fibrinogen had significantly statistical differences between the two groups （P 〈0.05）.In the two groups placental abruption,heart failure,HELLP syndrome,fetal distress,pre-mature and fetal growth restriction,stillbirth had statistical differences （P 〈0.05）.Conclusion The rela-tive factors of kidney function inj ury of severe Pre-eclampsia which is related with a variety of serious com-plications of the mother,fetus and newborn are systolic blood,incidence of gestational age,regular ob-stetric examination,serum album and Coagulation.
