超声弹性成像联合声触诊组织成像量化技术诊断乳腺肿瘤准确性研究

摘    要:目的:探讨超声弹性成像联合声触诊组织成像量化技术(VTQ)诊断乳腺良恶性肿瘤的准确性。方法选择118例乳腺肿瘤患者,术前行 VTQ及超声弹性成像检查,测量乳腺肿瘤内部的 VTQ值,并对肿瘤硬度进行评分,以术后病理结果为金标准,计算超声弹性成像联合声触诊组织成像量化技术诊断乳腺肿瘤良恶性的准确性。结果超声弹性成像、VTQ技术单独和联合使用对鉴别诊断乳腺肿瘤良恶性与病理结果比较,差异均有统计学意义(P <0.05)。ROC曲线显示,以硬度评分4分为临界值,超声弹性成像诊断乳腺肿瘤良恶性的灵敏性为92%,特异性为91%。以肿瘤内部 VTQ值3.91 m/s 为临界值,VTQ技术诊断乳腺肿瘤良恶性的灵敏性为85%,特异性为97%。超声弹性成像联合 VTQ技术诊断乳腺肿瘤良恶性的灵敏性为93%,特异性为95%。结论超声弹性成像联合 VTQ技术能提高诊断良恶性乳腺肿瘤的准确性。

Abstract:Objective To investigate the diagnostic accuracy of breast tumors by the virtual touch tissue quantification technique combined with ultrasound elasticity imaging.Methods 107 cases of breast tumors were enrolled in this study,the virtual touch tissue quantification technique and ultrasound elasticity imaging technique were performed at preoperative.The shear wave velocity (SWV)was measured by the VTQ in tumor center,and the grade of breast tumor hardness was valued by elasticity imaging technique.Using postoperative pathological results as the golden standard,the diagnostic accuracy of breast tumors by the virtual touch tissue quantification and ultrasound elasticity imaging were calculated.Results There was statistically significant in differential diagnosis of benign and malignant breast tumors by ultrasound elasticity imaging (P 〈0.05),and the ROC curve showed that with the hardness grade 4 as the critical value, the sensitivity was 92% and the specificity was 91% to diagnosis the benign and malignant breast tumor. There was statistically significant in differential diagnosis of benign and malignant breast tumors by VTQ (P 〈0.05),and the ROC curve showed that with the SWV 3.91 m/s as the critical value,the sensitivity was 85% and the specificity was 97%.There was statistically significant in differential diagnosis of benign and malignant breast tumor by VTQ combined with ultrasound elasticity imaging (P〈0.05),the sensitivity was 93%,and the specificity was 95%.Conclusion The diagnostic accuracy of breast tumors can be improved by the virtual touch tissue quantification combined with elasticity imaging.
