后路单开门铆钉内固定治疗多节段脊髓型颈椎病的疗效分析

目的：探讨颈椎后路单开门铆钉内固定治疗多节段脊髓型颈椎病的短期疗效。方法收集2011年1月-2013年3月新疆医科大学第六附属医院脊柱外科应用颈椎后路单开门铆钉内固定治疗多节段脊髓型颈椎病患者且获得随访的25例病例的临床资料。比较患者手术前后日本骨科学会JOA（Japanese Orthopedic Associa-tion）评分、颈椎功能障碍指数NDI（Neck Disability Index ），评估其术后神经功能障碍恢复及生活工作改善情况。结果应用颈椎后路单开门铆钉内固定治疗多节段脊髓型颈椎病，患者术后运动、感觉及疼痛恢复良好，未见明显术后并发症的发生，JOA评分由术前的（7．48±0．53）分增加至术后的（12．84±0．85）分，NDI评分由术前的（62．5±4．1）分降至术后的（46．4±2．2）分，在随访期间术后JOA、NDI评分与术前相比差异均有统计学意义（P ＜0.05）。结论后路单开门铆钉内固定治疗多节段脊髓型颈椎病的短期临床疗效较为满意，但是长期疗效尚待进一步观察。

The efficacy analysis of posterior cervical unilateral open-door expansive laminoplasty rivet for multilevel cervical spondylotic myelopathy

Objective To explore the short-term clinical efficacy of posterior cervical unilateral open-door expansive laminoplasty rivet for multilevel cervical spondylotic myelopathy .Methods A retrospective re-search was conducted on 25 cases with posterior cervical unilateral open-door expansive laminoplasty rivet for multilevel cervical spondylotic myelopathy from 2011 1.-2013 3. in No .6 Affiliated Hospital of Xinjiang Medical University to assess the post-operative recovery and improvement of the neurological dysfunction by comparing the Japanese Orthopaedic Association JOA （Japanese Orthopedic Association） score and the cervical dysfunction index NDI （Neck Disability Index ） pre-and-post the surgery .Results The patients with posterior cervical unilateral open-door expansive laminoplasty rivet for multilevel cervical spondylotic myelopathy had good recovery in the postoperative movement , sensation and pain relief , with no significant postoperative complications .JOA scores increased from preoperative （7 4.8 ± 0 5.3） to postopera-tive （12 8.4 ± 0 8.5）;NDI scores reduced from the preoperative （62 5. ± 4 1.） to postoperative （46 4. ± 2 2.） , with statistically significant difference in JOA scores and NDI scores of the post-operation compared with＆amp;nbsp;those of the pre-operation during the follow-up （P 〈0.05） .Conclusion The short-term clinical efficacy of posterior cervical unilateral open-door expansive laminoplasty rivet for multilevel cervical spondylotic my-elopathy was satisfactory ,but the long-term efficacy remained to be studied .
