安康市中心医院慢性阻塞性肺疾病住院患者用药分析

目的：了解我院慢性阻塞性肺疾病（COPD）患者的用药情况,评价其用药合理性,探讨该病在我院的药物基本治疗方案。方法：对我院呼吸科2012年1—4月诊断为COPD的病历进行回顾性分析,对抗菌药物、相关药物的使用情况进行统计、分析,依照《慢性阻塞性肺疾病诊治指南》评价我院呼吸科治疗COPD药物使用的合理性。结果：在调查的80例患者中,48例患者有吸烟史,常见并发症为肺心病;患者全都使用了抗菌药物,其中73例患者联合使用抗菌药物;消除呼吸道症状的主要药物有支气管扩张剂、糖皮质激素、祛痰药与中成药。结论：我院治疗COPD用药结构与＂指南＂基本一致,药品的使用基本合理,但仍存药物敏感性试验检查比例较低、药物使用剂量过大、超长时间使用抗菌药物、使用抗菌药物档次偏高、糖皮质激素的使用时间过长等问题,应加强合理用药。

Analysis of Drug Use in The Inpatients with Chronic Obstructive Pulmonary Disease in Ankang Central Hospital

OBJECTIVE：To probe into the drug use in the patients with chronic obstructive pulmonary disease（COPD） in our hospital and evaluate the rationality in drug use for COPD patients and discuss the basic therapeutic regimen for COPD in our hospital.METHODS：The medical records of patients who were diagnosed as having COPD by our respiratory department from January to April in 2012 were reviewed in respect of the use of antibacterials and other related medicines.The rationality of drug use in COPD patients in the respiratory department of our hospital was evaluated in accordance with the ＂Diagnosis and Treatment Guideline about COPD＂.RESULTS：Of the 80 cases reviewed,48 had smoking history;the common complication was pulmonary heart disease;all received antibacterials,and 73 had combined use of antibacterials.Bronchodilator,glucocorticoids,expectorant drugs and Chinese patent drugs were the predominant drugs for eliminating the respiratory symptoms.CONCLUSION：The medication structure for treating COPD in our hospital is basically in accordance with the Guideline ＂Diagnosis and Treatment Guideline about COPD＂ but with problems of low rate of susceptibility test,overdosage,prolonged use of antibiotics,high grade in antibiotic use and prolonged use of glucocorticoids.Rational drug use should be strengthened.
