高强度超声聚焦刀联合肿瘤坏死因子门静脉癌栓内注射治疗肝癌

目的：探讨高强度超声聚焦刀联合肿瘤坏死因子门静脉癌栓内注射治疗肝癌的有效性及安全性。方法：原发性肝癌（HCC）合并门静脉癌栓（PVTT）38例，其中高强度超声聚焦刀联合肿瘤坏死因子门静脉癌栓内注射治疗19例（A组），其余19例（B组）仅行超声聚焦刀治疗局部肿瘤。结果：A组门静脉内癌栓的消失率、缩小率、有效率分别为15．8％、52．6％、68．0％，而B组的门静脉癌栓消失及缩小率均为0，两组相比有显著性差异。A组患者的1、2、3年生存率为73．7％、42．1％、21．1％，而B组的1、2、3年的生存率为78．9％、10．5％、5．3％。两组的1年生存率无明显差别，而2、3年的生存率差别显著。结论：采用高强度超声聚焦刀联合肿瘤坏死因子门静脉癌栓内注射治疗肝癌效果显著。

Study of percutaneous rmhTNF injection into portal vein tumor thrombosis combine with HIFU in treatment of hepatocellular carcinoma

Objective ：To explore the safety and efficacy of inject ion of rmhTNF into portal vein tumor thrombosis guided by US combined with HIFU to treat hepatocellular carcinoma. Methods ：Thirty - eight cases of hepatocellular carcinoma （HCC） with PVTT were divided into two groups. A groups was administered percutaneous rmhTNF injection into portal vein（PPEI） tumor thrombosis and HIFU,and B group was treated only with HIFU. Results： The disappearing,shrinking and total efficacy rates of PVTT in group A were 15.8% ,52.6% ,68.0% respectively,but there was no change in group B. One -, two -, and three year survival rates were remarkably higher in group A than in group B,73.7% vs. 78.9% ,42.1% vs. 10.5% ,21.1% vs 5.3%, respectively. Conclusion： Injecting rmhTNF into portal vein tumor thrombosis guided by US combined with HIFU for treatment of hepatocellular carcinoma is a safe and effective therapeutic method.
