序贯有创-无创机械通气治疗慢性阻塞性肺疾病急性加重合并2型呼吸衰竭患者临床价值探讨

目的:探讨序贯有创-无创机械通气治疗慢性阻塞性肺疾病急性加重期(AECOPD)合并呼吸衰竭的治疗效果。方法:选择80例慢性阻塞性肺疾病合并呼吸衰竭患者,随机分为观察组和对照组。两组均给予常规治疗,观察组给予有创无创急性通气序贯治疗,对照组给予有创机械通气治疗。观察两组治疗效果。结果:观察组治疗后的血气分析结果分别与对照组治疗后比较,差异无统计学意义(P>0.05);观察组总的机械通气时间低于对照组、观察组住院时间低于对照组,观察组呼吸机相关肺炎发生率低于对照组,差异有统计学意义(P<0.05)。结论:序贯有创-无创机械通气治疗AECOPD合并2型呼吸衰竭临床效果显著,值得借鉴。

Clinical value of sequential invasive-noninvasive mechanical ventilation on AECOPD complicated with type two respiratory failure

Objective To explore the clinical effect of sequential invasive- noninvasive mechanical ventilation on AECOPD complicat- ed with type two respiratory failure. Method 80 cases with chronic obstructive pulmonary disease complicated with respiratory failure were selected and randomly divided into observation group and control group. Two groups were given conventional treatment, the observation group received sequential invasive- noninvasive mechanical ventilation, the control group were given invasive mechanical ventilation. The thera- peutic effect of two groups was observed. Results The blood gas analysis results in the observation group after treatment were compared with those in the control group after treatment,there was no significant difference （P 〉 0. 05 ） ;The total mechanical ventilation time in the obser- vation group was lower than that in the control group, the hospitalization time in the observation group was lower than that in the control group ,The ventilator associated pneumonia in the observation group was lower than that inthe control group, the difference was statistically significant （P 〈 0. 05 ）. Conclusion sequential invasive - noninvasive mechanical ventilation in the treatment of AECOPD complicated with respiratory failure has significant clinical effect, the therapy is worth learning.
