胸腔镜手术在孤立性肺结节诊治中的临床应用

目的研究孤立性肺结节（SPN）的临床特征及电视胸腔镜（VATS）手术对其诊治的意义。方法回顾性地分析57例施行VATS辅助手术的SPN患者的临床资料，比较SPN大小、位置分布与良恶性的关系；比较SPN中恶性结节大小与区域淋巴结转移的关系。结果57例SPN中恶性肿瘤38例（占66．7％）。SPN大小及位置分布与SPN性质无统计学意义（P〉0．05）。SPN中恶性结节的大小与区域淋巴节转移有统计学意义（P〈0．05）。结论SPN大小及位置分布并无良恶性鉴别诊断的价值。恶性结节的大小与区域淋巴节转移有关，VATS可同时诊断与治疗SPN，具有独特意义。

Clinical application of video-assisted thoracoscopic surgery in diagnosis and treatment of the solitary pulmonary nodule

[Objective] To study the clinical features and the significance of video-assisted thoracoscopic surgery in the diagnosis and treatment of solitary pulmonary nodule(SPN). [Methods] Clinical data of 57 patients with SPN who had received video-assisted thoracoscopic surgery were retrospectively analyzed. Compare the diameter of SPN and to determine wether the location of SPN related to the quality of the lesions; To determine wether the diameter of the malignant SPN related to the local lymphatic metastasis. [Result] 38 cases were diagnosed malignant(66.7%). There were no statistical significance between the diameter and location with the quality of SPN(P >0.05). There are statistical significance between the diameter of the malignant SPN with the local lymphatic metastasis(P <0.05). [Conclusion] The diameter and location cannot be used to differ the quality of the SPN(belign or malignancy).The diameter of the malignant node related to the local lymphatic metastasis, VATS may used to the diagnosis and treatment of SPN.
