生脉注射液对心肺复苏兔脑细胞的保护作用及应用时机研究

摘    要:[目的]探讨生脉注射液在心肺复苏中对兔脑细胞的保护作用及其应用时机。[方法]健康清洁级新西兰大白兔30只,采用窒息法制成心跳骤停动物模型,随机平均分为5组:A组为常规复苏组,B、C、D、E组分别在常规复苏同时、复苏5min、10min、15min时加用生脉注射液(SMI)5ml。观察各组动物自主心跳、呼吸恢复时间、自主呼吸恢复率、复苏成功率及脑细胞的病理和超微结构的改变,检测血清丙二醛(MDA)含量、SOD活力的变化。[结果]自主心跳、呼吸恢复时间B、C、D、E组均较A组短(P<0.01)。自主呼吸恢复率A组为66.67%,B、C、D组均为100%,E组为83.33%。复苏成功率各组均为100%。SOD活力在复苏过程中呈动态下降趋势,加用SMI可以延缓这一趋势。复苏60min时,加用SMI组SOD活力明显高于A组。MDA含量在复苏过程中呈动态增加,加用SMI可以延缓这一趋势。复苏60min时,加用SMI组MDA含量明显低于A组。复苏后各组脑细胞病理及超微结构均存在不同程度的改变,A组病变最严重,B组病变最轻,C组、D组、E组病变依次加重,但都比A组轻。[结论]加用生脉注射液辅助心肺脑复苏可以缩短复苏兔自主循环和自主呼吸恢复时间,提高自主呼吸恢复率,减少血清丙二醛(MDA)的产生,延缓SOD活力的下降,减轻复苏兔脑细胞病理及超微结构的改变,对脑细胞具有保护作用,且与应用时机有一定的关系,应用越早,其作用越明显。

Abstract:[Objective]To study the protective effects of ShengMai Injection on cardiopulmonary resuscitation of rabbit brain cell and its applied time.[Methods]Thirty Zealand rabbits were made into cardiac arrest models by smothering method, and then randomly divided into five groups. Except the control group (A), 5ml ShengMai Injection was given to the different group every five min. Then the observation was made to see the time of restoration of spontaneous circulation and breath, the CPR rate, and the changes of pathology and ultra microstructure in myocardial cell. Meanwhile, the malondialdehyde (MDA) contents and the superoxide dismutase (SOD) activity in the Serum were measured.[Results]The time of restoration of spontaneous circulation and breath in the group A was longer than that in the other four groups (P<0.01). The recovery rate of spontaneous breath was 66.67% in the group A, 83.33% in the group E, and 100 % in the other three groups; while the resuscitation rate of CPR in each group achieved 100%. 60 min after resuscitation, less MDA contents and higher SOD activities were found in the four ShengMai Injection groups. The increase of MDA contents and decrease of SOD activity during the cardiopulmonary resuscitation were postponed by ShengMai Injection. The pathology and ultra microstructure in myocardial cell were changed in different degree with the group A being the most, then the group E, D, C, and the Group B being the lightest.[Conclusion]ShengMai Injection can shorten the time of restoration of spontaneous circulation and breath, reduce the production of MDA contents, and improve the SOD activity in rabbits Serum. It also can ease the damages in both the pathology and ultra microstructure in myocardial cell and protect the brain cells. Moreover, better curative effects can be achieved if it is applied earlier.
