巨大腹膜后原发恶性肿瘤的外科处理

目的 探讨巨大腹膜后肿瘤手术切除的方法，提高手术安全性。方法 回顾性分析我院1990—1991年收治的X例巨大腹膜后患者肿瘤患者的临床资料，重点分析术前准备和手术技巧。结果 多数患者顺利度过围手术期，1例术后死亡，1例出现胆瘘、胰瘘，3例出现肺炎，3例伤口液化。结论 为提高巨大腹膜后肿瘤切除的安全性和彻底性，术前准备要充分，术中要正确处理大血管和联合切除受累脏器。

Surgical management of huge primary malignant retroperitoneal tumor

Objective To investigate the methods of surgical resection in huge primary malignant retroperitoneal tumor and improve the safety. Methods A retrospective study was performed in 25 cases with huge primary malignant retroperitoneal tumor from 1990-2001. Our focal point included preoperative preparation and surgical technic. Results Most of the patients smoothly passed the period of perioperation. One case died after operation.Biliary fistula and pancreatic fistula occurred in one case. Pneumonia occurred in 3 cases. Wound liquefaction occurred in 3 cases. Conclusion To increase the total resection rate and improve the safety, more attention should be paid to the preoperative preparation. And it is important to resect the involved big vessel and organ together.
