间苯三酚对子痫前期患者分娩过程中血浆皮质醇及血糖/胰岛素影响初探

目的 探讨间苯三酚对子痫前期患者血浆皮质醇及血糖/胰岛素的影响,及其对产程的促进效果.方法 选择无产科指征初产子痫前期患者80例,随机均分为观察组、对照组.观察组活跃期宫口开大3 cm,给予间苯三酚80 mg静脉注射,30 min后再次给予间苯三酚80 mg静脉注射,测定两组产妇血浆皮质醇及血糖与胰岛素比值,观察两组产妇产程变化、胎儿情况及围产结局.结果 当宫口开大3 cm时,观察组皮质醇为(45.32±15.36)μg/dL,血糖/胰岛素为0.37±0.21;对照组皮质醇为(42.69±12.57) μg/dL,血糖/胰岛素为0.43±0.32.宫口开全时,观察组皮质醇为(53.61±12.35) μg/dL,血糖/胰岛素为0.54±0.32;对照组皮质醇为(51.64±13.56)μg/dL,血糖/胰岛素为0.46±0.35.经统计学分析,观察组与对照组血浆皮质醇、血糖/胰岛素差异无显著性(P＞0.05).结论 子痫前期患者静推间苯三酚,对产妇正常应激反应无明显影响.

THE EFFECT OF PHLOROGLUCINOL ON PLASMA CORTISOL AND THE RATIO OF GLUCOSE/INSULIN IN THE PROCESS OF DELIVERY IN PAITENTS WITH PREECLAMPSIA

Objective To study the effects of phloroglucinol on plasma cortisol and the ratio of glucose/insulin in the process of delivery in patients with preeclampsia.Methods 80 patients with preeclampsia and no obstetric indications with them were chosen in the study.The cases were randomly divided into observation group and control group.The observation group received intravenous injection of Phloroglucinol(30mg)when cervical mouth open 3 cm in active period,then 80mg of Phloroglucinol was injected intravenously again.The ratio of plasma cortisol,glucose/insulin,the process of delivery and the condition of the fetus and perinatal outcome were detected and observed.Results When the cervical mouth open 3cm,the level of plasma cortisol was(45.32 ±15.36)μg/dL and the ratio of glucose/insulin was 0.37±0.21 in observation group;whereas the level of plasma cortisol was(42.69±12.57)μg/dL and the ratio of glucose/insulin was 0.43±0.32 in contorl group.When the cervical mouth was fully opened,the level of plasma cortisol was(53.61±12.35)μg/dL and the ratio of glucose/insulin was 0.54±0.32 in observation group;whereas the level of plasma cortisol was(51.64±13.56)μg/dL and the ratio of glucose/insulin was 0.46±0.35 in control group.There were no significant difference between plasma cortisol,ratio of glucose/insulin the both groups.Conclusion Phloroglucinol can remarkably improve cervical dilation and decrease labor period,and has no obvious side-effect both in puerperas and fetus.
