肿瘤标志物AFP与CEA的联合检测在肝癌诊断中的临床价值

目的探讨肿瘤标志物甲胎蛋白(AFP)和癌胚抗原(CEA)联合检测在肝癌诊断中的临床价值。方法用电化学发光免疫分析法检测65例肝癌患者和78例正常对照者AFP、CEA血清水平并计算阳性率。结果肝癌组AFP、CEA水平显著高于对照组,差异有统计学意义(P<0.05);38例原发性肝癌(PHC)患者AFP水平与27例转移性肝癌(MHC)患者AFP水平比较,差异有统计学意义(P<0.05);PHC患者CEA水平与MHC患者CEA水平比较差异有统计学意义(P<0.05)。AFP在PHC组、MHC组阳性检出率分别为78.90%、22.20%;CEA阳性检出率分别为34.20%、70.40%;AFP、CEA联合检测使PHC组、MHC组阳性检测率分别提高至84.20%、77.80%。结论联合检测AFP和CEA在肝癌的诊断与鉴别诊断中具有较高的临床价值。

Clinical value of the combined detection of tumor markers AFP and CEA in the diagnosis of liver cancer

Objective To evaluate the clinical value of the combined detection of tumor markers alpha-fetoprotein（AFP） and carcinoembryonic antigen（CEA） in the diagnosis of liver cancer.Methods Serum AFP and CEA levels were detected by electrochemical luminescence immunoassay and the positive rates were calculated in 65 patients with liver cancer（primary liver cancer（PHC）,N= 38;metastatic liver cancer（MHC）,N=27） and 78 normal subjects.Results Compared with normal subjects,AFP and CEA levels were significantly higher（P0.01） in patients with liver cancer.There were significant differences in the levels of AFP and CEA between PHC and MHC patients.（P0.01,respectively）.AFP positive rates were 78.9% and 22.2%,CEA positive rates were 34.2% and 70.4% in PHC and MHC patients respectively;the positive rates of combined detection of AFP and CEA increased to 84.2 % and 77.8% in PHC and MHC patients respectively.Conclusion Combined detection of AFP and CEA has great clinical value in the diagnosis and differential diagnosis of liver cancer.
