重症监护病房呼吸机相关肺炎的临床分析

摘    要:目的分析重症监护病房呼吸机相关肺炎(VAP)病原菌特点及耐药性,便于控制VAP。方法对2010年4月至2011年3月重症医学科确诊的VAP患者分离到的病原菌进行鉴定并用纸片扩散法进行药物敏感试验。结果 132例机械通气的患者发生VAP 61例(46.21%);共分离出157株病原菌。其中,革兰阴性菌占68.9%,革兰阳性菌占26.1%,真菌占5.0%。位于前6位的病原菌依次为铜绿假单胞菌、鲍曼不动杆菌、大肠埃希杆菌、肺炎克雷伯菌、金黄色葡萄球菌及白色念珠菌。亚胺培南对革兰阴性菌有很好的敏感性。结论革兰阴性菌是重症监护病房VAP的主要病原菌,且存在严重耐药现象。临床医师应对VAP制定全面的预防策略并进行综合管理,同时根据药敏试验结果合理使用抗生素,及时、有效地治疗VAP。
