彩色多普勒超声鉴别诊断下肢动脉血栓的新旧程度

目的判断分析下肢动脉血栓的新鲜程度和陈旧程度,评价彩色多普勒超声对其诊断价值,为临床提供有效的治疗方案。方法应用彩色多普勒超声对下肢动脉进行检查,根据血栓的范围大小、回声强度及血管侧支循环的形成、血栓近端及远端频谱,进行定量分析,评价与血栓新旧程度的关系,并按其程度分成Ⅲ级,为临床治疗提供必要的指导信息。结果本组59例70支下肢血管血栓均经彩色多普勒超声等明确诊断,血栓的新旧程度在病程、管壁结构改变及有无侧支循环建立方面有差别,两组病变近端、远端最高血流速度和近端阻力指数有明显的差异(P<0.05),远端阻力指数无明显差异(P>0.05)。结论彩色多普勒超声是下肢动脉血栓新旧程度鉴别诊断的最佳的无创检查方法。

The Differential Diagnosis of Color Doppler Ultrasound to the New or Old Degree of Lower-limb Arterial Thrombosis

Objective To analyze the new and old degree of lower-limb arterial thrombosis,and evaluate the diagnostic value of color Doppler ultrasound for providing effective clinical treatment programs.Method The lower-limb artery of patients were examined with color Doppler ultrasound,and quantitatively analyzed the range of thrombus,the echo intensity,vascular formation of collateral circulation,thrombus proximal and distal spectrum,to evaluate the relationship of them with the degree of new and old thrombus which was divided three levels.Results The thrombus in 59 patients of 70 lower-limbs vascular was diagnosed definitely by color Doppler ultrasound.There was a difference in the disease course,structure of vascular wall and collateral circulation between the new thrombus and the old thrombus.There was definitely difference in the most blood flow speed of proximal terminal and distal terminal resistance index(P<0.05).There was no difference in the distal terminal resistance index(P>0.05).Conclusions The color Doppler ultrasound is the best non-wound examination of differential diagnosis in the new or old degree of lower-limb thrombus.
