肝脏肿物超声引导经皮细针吸取细胞学与组织学检查对照研究

本文收集1986年以来在B超引导下经皮细针吸取细胞学诊断肝脏肿物并具有组织学针吸活检的102例进行对比研究。结果证明:细胞学检查敏感性及特异性分别为93.1％及100％;而组织学活检分别为87.1％及100％。本检查对B超发现肝占位性病变而α-FP检查阴性者具有特殊意义。由于多为α-FP阴性病例,故以分化好的肝癌、肝母细胞瘤、转移癌及肉瘤等占有较高比例。这类病例单凭细胞学诊断存在一定困难、而组织学检查则常能弥补其不足。唯后者取材失败率较高,细胞学诊断效果较好。如能二者结合互补后,阳性率可近于100％。

Comparative Study of Cytologic and Histologic Examination of Liver Mass by Ultrasound-guided Percutaneous Fine-Needle Aspiration

From 1980 to 1988, 562 patients clinically suspicious of malignant hepatic disease underwent percutaneous fine- needle aspiration under ultrasonic guidance. Of these, 102 cases were diagnosed by histologic examination of tissue frag- ment simultaneously. A comparison was made between these two series. Overall accuracy rate of cytologic evaluation was 96.9% with a sensitivity rate of 93.1%, a specificiW rate of 100%. The accruacy rate of histopathologic diagnosis was 91.8%, 87.1%and 100% respectively. Percutaneous fine-needle aspiration guided by ultrasonics was proved to be a safe, useful and highly accurate technique for making cyto-histologic diagnosis of hepatic mass, especially in patients with negative AFP test.
