预防性围术期麻醉管理对老年手术患者麻醉并发症的影响研究

目的探讨围术期麻醉管理对老年手术患者麻醉并发症的影响。方法选择2009年11月至2012年11月收治的进行手术麻醉的老年患者120例,随机分为两组,对照组给予常规麻醉方法,观察组给予预防性围术期麻醉管理,对比两组患者麻醉并发症的情况。结果全部患者均顺利完成麻醉和手术,无死亡患者。观察组麻醉中发生血压下降、血压升高、心率失常、寒颤、术后心血管事件、低氧血症等麻醉并发症显著低于对照组,差异有统计学意义（P均〈0.05）。观察组麻醉并发症发生率32.5%,显著低于对照组87.5%,差异有统计学意义（P〈0.05）。结论对老年患者麻醉意外的各种可能和危险进行评估、界定,并对老年人的手术麻醉加强管理,可以最大限度地避免不良事件的发生,显著降低麻醉并发症。

The Prophylactic Perioperative Anesthetic Management of the Elderly Surgical Patients about Anesthe- sia Complications

Objective To investigate the perioperative anesthetic management of elderly patients about anesthesia complications. Methods 120 cases of elderly patients admitted to the hospital from November 2009 to November 2012 for surgical anesthesia were randomly divided into two groups. The control group were given general anesthesia, observation group were given prophylactic perioperative anesthesia management, anesthesia complications were compared between the two groups of patients. Results All patients were successfully anes- thesia and surgery, there was no deaths. Drop in blood pressure, elevated blood pressure, abnormal heart rhythms, chills, postoperative cardiovascular events, hypoxemia and anesthetic complications occurred in the observation group were significantly lower than those in the control group （ P 〈 0. 05 ）. The anesthesia complica- tion rate of observation group was 32. 5%, which was significantly lower than 87.5% of the control group, there was a significant difference （P 〈 0.05）. Conclusion In the elderly patient, anesthesia accident possibilities and risks were evaluated, defined, and for older people, to strengthen the management of operation of anesthe- sia can utmost ground to avoid the occurrence of adverse events and reduce complications of anesthesia.
