老年男性糖尿病合并痛风患者胰岛素敏感性指数及血浆ET水平的临床研究

摘    要:目的 探讨老年男性 2型糖尿病合并痛风患者胰岛素敏感性指数及血浆ET水平的变化。方法 对 22例 2型糖尿病合并痛风患者、21例无痛风的 2型糖尿病患者和 16例痛风患者进行空腹血糖 (FBG)、胰岛素 (FINS)、餐后 2h血糖、胰岛素、糖化血红蛋白(GHbA1C)、血尿酸(Ua)及空腹血浆ET测定,并计算胰岛素敏感性指数 (IAI)。结果 2型糖尿病合并痛风患者较无痛风的糖尿病患者具有明显的高胰岛素血症、IAI显著降低 (P<0 05 ),同时血浆ET水平较无痛风的糖尿病患者明显升高(P<0 05)。结论 2型糖尿病合并痛风患者具有明显高胰岛素血症和胰岛素抵抗。糖尿病合并痛风患者ET水平升高者较糖尿病无痛风患者更具慢性血管病变的倾向。

Abstract:Objective To investigate the changes of insulin resistance and plasma endothelin level in elder male diabetic patients with arthrolithiasis. Methods The level of fasting plasma glucose, fasting insulin, two hour postprandial glucose, two hour postprandial insulin, glycosylated hemoglobin, plasma uraic acid and fasting plasma endothelin were determined in 22 diabetic patients with arthrolithiasis. 21 diabetic patients without arthrolithiasis and 16 uratic patients then calculate the insulin sensitive index. Results Type 2 diabetic patients with arthrolithiasis had conspicuous hyperinsulinemia, and the level of ISI in diabetic patients with arthrolithiasis was significantly lower than that in diabetic patients without arthrolithiasis ( P <0 05). The plasma endothelin level in diabetic patients with arthrolithiasis was significant higher than that in diabetic patients without arthrolithiasis ( P <0 05). Conclusion Type 2 diabetic patients with arthrolithiasis have conspicucous hyperinsulinemia and insulin resistance. Patients with of high ET level in type 2 diabetes have a more tendency to chronic blood vessel complication than those without arthrolithiasis.
