HSP gp96和CEA在结直肠癌中的表达及临床意义的研究

摘    要:目的 通过检测结直肠癌组织及相应癌旁组织中的热休克蛋白糖蛋白96(Heat Shock Protein glycoprotein 96,HSP gp96)和癌胚抗原(Carcino Embryonic Antigen,CEA)的表达,探讨HSP gp96和CEA在结直肠癌组织中的表达及临床意义.方法 应用免疫组织化学方法检测88例结直肠癌及相应癌旁组织中的HSP gp96和CEA的表达情况.结果 HSP gp96在结直肠癌组织中的表达率明显高于癌旁组织(χ2=47.400,P<0.001),在低分化型癌中的表达高于中、高分化型(χ2=4.136,P=0.042),淋巴结转移阳性的病例中表达高于淋巴结转移阴性的病例(χ2=6.029,P=0.014),Dukes 分期C期+D期中表达高于A期+B期(χ2=6.820,P=0.009),HSP gp96的表达与患者性别、年龄、肿瘤大小无明显统计学意义(P>0.05);CEA在结直肠癌组织中的表达率明显高于癌旁组织(χ2=52.391,P<0.001),淋巴结转移阳性的病例中表达高于淋巴结转移阴性的病例(χ2=4.026,P=0.045),Dukes 分期C期+D期中表达高于A期+B期(χ2=5.640,P=0.018),CEA的表达与患者性别、年龄、肿瘤大小及分化程度无明显统计学意义(P>0.05);且 HSP gp96和CEA的表达存在关联(χ2=9.374,P<0.05,r=0.326).结论HSP gp96和CEA共同参与了结直肠癌的发展、侵润及转移;HSP gp96可作为结直肠癌新的肿瘤标志物.

Expression and clinic significance of HSP gp96 and CEA in colorectal carcinoma

Abstract:Objective To explore the expression and clinical significance of HSP gp96 and CEA in colorectal cancer,and to detect the expression of them in colorectal cancer tissues and corresponding adjacent tissues.Methods Immunohistochemistry was used to detect the expression of HSP gp96 and CEA in 88 cases of colorectal cancer and corresponding adjacent tissues.Results Positive expression rate of HSP gp96 in colorectal cancer was significantly higher than that in the adjacent tissues(χ2=47.400,P<0.001);positive expression of HSP gp96 in poorly differentiated carcinoma was higher than that in moderately and well differentiated carcinoma(χ2=4.136,P=0.042);the expression rate of HSP gp96 in positive lymph nodes metastasis was higher than that in negative lymph nodes metastasis(χ2=6.029,P=0.014);positive HSP gp96 expression of C stage+D stage of Dukes classification was higher than that of A stage+B stage(χ2=6.820,P=0.009).There were no statistically significant differences between the expression of HSP gp96 and gender,age and tumor size of colorectal cancer(P>0.05).The expression of CEA in colorectal cancer tissues was significantly higher than that in the adjacent tissues(χ2=52.391,P<0.001);the expression rate of CEA in positive lymph nodes metastasis was higher than that in negative lymph nodes metastasis(χ2=4.026,P=0.045),positive CEA expression of C stage+D stage of Dukes classification was higher than that of A stage+B stage(χ2=5.640,P=0.018);there were no statistically significant differences between the expression of CEA and gender,age,tumor size and degree of differentiation of colorectal cancer(P>0.05).Moreover,there was positive correlation between the expression of HSP gp96 and CEA(χ2=9.374,P<0.05,r=0.326).Conclusion HSP gp96 and CEA participate in development and metastasis of colorectal cancer;HSP gp96 may be a new tumor marker of colorectal cancer.
