13例阴茎癌临床诊治体会

目的:总结临床病例,旨在提高对阴茎癌的诊治认识。方法:对我院2002年10月～2008年5月收治的13例阴茎癌患者的临床资料进行回顾性分析,行阴茎部分切除术11例,阴茎全切并会阴部造口术2例,行双侧腹股沟淋巴结清扫术4例,术后辅助放疗1例。结果:术后病理结果:高分化鳞状细胞癌10例,低分化鳞状细胞癌2例,鳞状上皮乳头状瘤1例。4例行淋巴清扫术者2例证实淋巴结转移,2例为炎性增生。术后1例发生尿道外口狭窄,1例出现单侧腹股沟区皮瓣部分坏死。术后13例获得随访1～5年,2例因出现广泛转移于术后1年死亡,其余患者存活至今。结论:阴茎部分切除术是治疗阴茎癌的有效方法,对合并淋巴结转移者早期行淋巴结清扫术可提高疗效。

Experience in the diagnosis and treatment of the carcinoma of penis: report of 13 cases

Objective：To summarize experience in the diagnosis and treatment of carcinoma of penis.Method：Clinical data of 13 cases of carcinoma of penis were reviewed,of which 11 cases underwent partial amputation of the penis,2 cases received total penis amputation plus perineal urethrostomy;4 cases experienced inguinal lymph nodes dissection and 1 had neoadjuvant radiotherapy.Results：Pathological reports showed that 10 cases were well-differentiated squamous carcinoma,2 were poorly differentiated squamous-cell carcinoma and 1 was squamous papilloma.Two of the 4 patients unwent ilioinguinal lymphadenectomy were confirmed as lymphatic metastasis,while 2 were inflammatory hyperplasia.Postoperative complications included narrowed orificium urethrae externum in 1 case and partial necrosis in the iliac region in the other.One to five years follow up conducted on 13 cases showed all patients survived except two deaths due to extensive lymph nodes metastasis.Conclusions：Partial penectomy is an effective treatment for penis carcinoma and correct inguinal lymph nodes dissection in the early stage can improve the patients＇ survival time.
