64层螺旋CT诊断不同程度冠状动脉狭窄的临床价值

目的探讨64层螺旋CT冠状动脉成像对诊断冠状动脉狭窄程度的临床价值。方法将我院2011年9月至2012年9月收治的60例疑似冠心脏病患者采用64层螺旋CT冠状动脉成像诊断,并与常规的冠状动脉造影诊断的结果进行比较,观察64层螺旋CT诊断不同程度冠状动脉狭窄的敏感度、特异性及诊断正确率等。结果 64层螺旋CT诊断轻度、中度以及重度冠状动脉狭窄的敏感度分别为68.6%、81.0%和90.9%;诊断的特异性分别为99.0%、99.3%和98.0%,诊断正确率分别为94.5%、97.1%和96.2%。诊断一致性较好,Kappa=0.453。结论采取64层螺旋CT诊断不同程度的冠状动脉狭窄具有重要的临床应用价值,诊断正确率高。

The clinical value of 64-slice CT in the diagnosis of different degrees of coronary artery stenosis

Objective To investigate the clinical value of 64-slice CT in the diagnosis of coronary artery stenosis.Methods Sixty patients with suspected coronary heart attack admitted in our hospital from September 2011 to September 2012 took the 64-slice CT coronary angiography in the diagnosis,and the diagnosis results were compared with those of conventional coronary angiography.The sensitivity,specificity,and diagnostic accuracy rate of 64 slice spiral CT diagnosis in varying degrees of coronary stenosis were observed.Results By comparison with the results of conventional coronary angiography in the diagnosis,the sensitivity of 64-slice spiral CT diagnosis in mild,moderate and severe coronary artery stenosis was 68.6%,81.0%,and 90.9%,respectively,the specificity of the diagnosis was 99.0%,99.3%,and 98.0%,respectively,and the diagnostic accuracy was 94.5%,97.1%,and 96.2%,respectively.There was satisfactory diagnostic agreement（Kappa=0.453）.Conclusion 64-slice spiral CT for diagnosing different degrees of coronary artery stenosis has important clinical value and high diagnostic accuracy rate.
