经伤椎固定治疗胸腰段脊柱骨折的效果和安全性分析

目的分析经伤椎固定治疗胸腰段脊柱骨折的效果和安全性，以探讨其临床适用性。方法选取2011年4月～2013年5月在本院接受治疗的48例胸腰段脊柱骨折患者作为研究对象，随机分为观察组和对照组，对照组采用跨伤椎固定方案进行治疗，观察组采用经伤椎固定方案进行治疗，观察并比较两组的治疗前后Cobb角、术后即刻矫正度、治疗6个月后矫正丢失度及治疗前后伤椎前后缘高度。结果两组的治疗前Cobb角比较，差异无统计学意义（P〉0．05）；观察组的治疗后Cobb角、术后即刻矫正度、治疗6个月后矫正丢失度均明显优于对照组，差异有统计学意义（P〈0．05）；观察组的治疗后相对高度、术后即刻矫正度、治疗6个月后矫正丢失度的伤椎前后缘高度均明显优于对照组，差异有统计学意义（P〈0．05）。结论经伤椎固定治疗胸腰段脊柱骨折效果良好，安全可靠，值得临床推广应用。

Effect and safety analysis of fixation via the injured vertebra in the treatment for thoracolumbar spine fracture

Objective To analyze effect and safety analysis of fixation via the injured vertebra in the treatment for tho- racolumbar spine fracture in order to investigate the clinical applicability. Methods 48 cases of patients with thora columbar spine fracture from April 2011 to May 2013 in our hospital were selected as research objects and randomly divided into observation group and control group.The control group were treated with across the injured vertebra fixation treatment,the observation group were treated with via the injured vertebra fixation treatment.Before and after treatment of Cobb angle,postoperative immediate correction angle,loss correction angle after 6 months of treatment,injured verte- bral anterior and rear height in two groups was observed and compared respectively. Results Before treatment of Cobb angle in two groups was compared,with no statistical differenee（P〉0.05）.After treatment of Cobb angle,postoperative immediate correction angle,loss correction angle after 6 months of treatment in observation group was significantly better than that in control group was respectively,with statistical difference （P〈0.05）.Injured vertebral anterior and rear height of after treatment of relative height,postoperative immediate correction angle,loss correction angle after 6 months of treatment in observation group was significantly better than that in control group respectively,with statistical difference （P〈0,05）. Conclusion The effect of fixation via the injured vertebra in the treatment for thoracolumbar spine fracture is safe and reliable,is worthy of promotion and application in clinic.
