低分子肝素钠在腹部外科手术后预防下肢深静脉血栓形成的效果观察

目的探讨低分子肝素钠在腹部外科手术后预防下肢深静脉血栓形成的效果。方法选取2008年5月～2012年9月在本院外科行腹部手术的136例患者，随机分为治疗组68例。对照组68例，治疗组于术后24h开始使用常规剂量的低分子肝素钠，同时予以常规预防及护理，对照组术后采用常规预防及护理，比较两组下肢深静脉血栓的发生率，观察两组治疗前后血小板计数的变化，记录两组治疗后的纤维蛋白原、凝血酶原时间及活化部分凝血活酶时间。结果治疗组术后下肢深静脉血栓发生率为0．0％，对照组下肢深静脉血栓发生率为22．1％．差异有统计学意义（P〈0．05）。两组治疗前后血小板计数比较差异无统计学意义（P〉0．05）。两组治疗后纤维蛋白原、凝血酶原时间及活化部分凝血活酶时间比较差异无统计学意义（P〉O．05）。结论低分子肝素钠对预防腹部外科手术后下肢深静脉血栓形成有很好的效果。

Efficacy observation of low molecular weight heparin in prevention of deep vein thrombosis after abdominal surgery

Objective To investigate the efficacy of low molecular weight heparin in prevention of deep vein thrombosis after abdominal surgery. Methods 136 cases of patients carried out abdominal surgery in our hospital from May 2008 to September 2012 were selected and randomly divided into treatment group of 68 patients and control group of 68 patients,the treatment group was treated with conventional doses of low molecular weight heparin after 24 hours surgery and were given with preventive and routine care.The control group wes given with conventional prevention and care af- ter surgery.The incidence rate of deep vein thrombosis was compared between two groups,change in platelet count was observed before and after treatment in two groups,after treatment,fibrinogen,prothrombin time and activated partial thromboplastin time in two groups was recorded respectively. Results After treatment,the incidence rate of deep vein thrombosis in treatment group was 0.0%,the incidence rate of deep venous thrombosis in control group was 22.1%,with statistical difference（P〈0.05）.Platelet counts before and after treatment showed no statistical difference in two groups（P〉 0.05）.After treatment,fibrinogen,prothrombin time and activated partial thromboplastin time of two groups had no statistical difference （P〉0.05）. Conclusion The low molecular weight heparin has a good effect in prevention of deep vein thrombosis after abdominal surgery.
