Comparison of curative effect of the treatment of ankle fractures by way of closed reduction and percutaneous hollow screws fixation and way of open reduction and internal fixation

Objective To compare the curative effects of ankle fractures treated by the way of closed reduction and percutaneous hollow screw internal fixation and way of traditional open reduction and internal fixation.Methods Totally 98 cases of non-comminuted ankle fracture admitted to our department from Mar.2004 to Aug.2009 were retrospectively analyzed.According to the different internal fixation methods,the patients are divided into closed reduction and percutaneous hollow screw internal fixation(group MINI) and traditional open reduction and internal fixation group(group ORIF).The group MINI consisted of 53 patients,including 42 males and 11 females,with the average age of 37.5 years;Weber-AO fracture classification: 6 cases of type A,45 cases of type B,2 cases of type C.Group ORIF consisted of 45 patients,including 37 males and 8 females,with the average age of 36.4 years;Weber-AO fracture classification: 5 cases of type A,39 cases of type B,1 case of tpye C.The operative time,blood loss,fracture healing time,lateral ankle pain and the score of AOFAS of the last follow-up were compared.Results Two cases of group MINI were unavailable to follow-up,and the other healed 51 cases had an average follow-up of 29.7 months(16-81 months).Three cases of group ORIF were unavailable to follow-up,and the other 42 cases had an average follow-up of 28.3 months(17-80 months),which indicated 1 case of bone nonunion.The operative time,blood loss,fracture healing time and fracture healing without lateral ankle pain in group MINI were obviously better than those of group ORIF.The difference was statistically significant(P<0.05).Conclusion Compared with the traditional open reduction and internal fixation,closed reduction and percutaneous hollow screw internal fixation for ankle fractures has advantages of shorter operative time,less blood loss,faster bone union,less lateral ankle pain after bone union,which can achieve the similar ankle joint function treated by the traditional open reduction and internal fixation.Closed reduction and percutaneous hollow screw internal fixation is a feasible treatment of ankle fracture.
