榄香烯乳注射液联合放疗治疗恶性脑胶质瘤的临床观察

目的　探讨榄香烯乳注射液与放疗同步治疗恶性脑胶质瘤的临床疗效。方法　回顾分析我科２００４年至２００６年恶性脑胶质瘤术后残留或复发的患者３４例给予榄香烯乳注射液与放疗同步治疗的临床疗效。放疗采用６ＭＶ-Ｘ电子直线加速器，ＤＴ５０～５６Ｇｙ，１８０～２００ｃＧｙ／次，５次／周。榄香烯乳注射液静脉滴注８００ｍｇ／日，贯穿整个治疗过程，总量为２８ｇ。对照组２６例单纯放射治疗，治疗方法同前。结果　（１）观察组获ＣＲ４例，ＰＲ２０例，有效率（ＣＲ＋ＰＲ）为７０.６％（２４／３４）；对照组获ＣＲ２例，ＰＲ１１例，有效率（ＣＲ＋ＰＲ）为５０.０％（１３／２６），两组差异有统计学意义（Ｐ＜０.０５）。（２）观察组和对照组１、３年生存率分别为６４.７％、４１.２％和４２.３％、１５.４％，两组差异有统计学意义（Ｐ＜０.０５）。（３）治疗组榄香烯乳注射液副作用主要为发热，发生率为８.８％（３／３４），两组放疗合并轻度脑水肿发生率无显著差异，两组血液学毒性轻微，亦无显著差异。结论　榄香烯乳注射液联合放疗治疗恶性脑胶质瘤术后残留或复发患者的疗效好，生存期延长，毒副反应未见明显增加，值得进一步临床探讨。

Clinical observation of elemene injection combined with radiotherapy for patients with malignant glioma

Objective To investigate the effect of elemene injection combined with radiotherapy for brain glioma. Methods Form 2004-2006,34 patients with brain glioma had received elemene injection combined with radiotherapy were retrospectively reviewed. According to lesion location, the scope of tumor resection ,these patients received a dose of 50-56Gy/180-200cGy/5 weeks by 6mv-x ray,5 times a week. At the same time, elemene was treated throughout the entire process by 800 mg one day in vein. In control group, 26 patients was treated with radiotherapy alone, and the treatment was the same to the former. Results In the observation group ,4 cases achieved complete response ,20 cases had partial response, the total response rate was 70.6%, while in the control group, 2 cases achieved complete response, 11 cases had partial response, the total response rate was 50. 0% （ P 〈 0. 05 ）. The 1 -, 3-year survival rates were 64. 7% and 41.2% in the observation group and 42. 3% and 15.4% in the control group. The side effect of elemene injection was fever with a rate of 8. 8% ,but the rates of cerebral edema and hematologic toxicity were similar in both groups, Conclusion Elemene injection combined with radiotherapy is effective for brain glioma and has a longer survival time, without increasing the side effects of radiotherapy.
