微创胸腔镜下纵膈切除术治疗纵隔肿瘤的疗效

目的:探讨微创胸腔镜手术治疗纵膈肿瘤的疗效,减少术后并发症. 方法:对2014年1月至2015年6月收治的纵膈肿瘤患者40 例(研究组)前瞻性实施微创电视胸腔镜下纵膈切除术治疗,并回顾性分析2012年7月至2013年12月实施开胸手术治疗40例纵膈肿瘤患者(对照组)的临床资料,比较两组手术情况及并发症发生率. 结果:两组手术时间比较差异无统计学意义( P>0.05);研究组术中失血量,引流管留置、下床活动、住院时间均少于对照组( P<0.05);术后1d、术后3d研究组切口疼痛评分(VAS),血浆中皮质醇(COR)、促肾上腺皮质激素(ACTH)浓度均低于对照组(P<0.05);研究组并发症发生率低于对照组( P<0.05). 结论:微创胸腔镜纵膈切除术治疗纵膈肿瘤疗效好,创伤小,患者应激反应低,并发症少,临床应推广适用.

Efficacy of Minimally Invasive Thoracoscopic Mediastina Resection in the Treatment of Mediastinal Tumor

Objective:To investigate the effect of of minimally invasive thoracoscopic mediastina resec-tion in the treatment of mediastinal tumor and reduce complications. Method: 40 patients with mediastinal tumors ( study group ) from January 2014 to June 2015 were prospectively implemented minimally invasive thoracoscopic mediastina resection,and clinical data of 40 patients with mediastinal tumors ( control group) from July 2012 to 2013 implemented thoracotomy surgury were analyzed, then surgery and complications inci-dence were compared. Result:Operative time had no significant difference ( P>0.05);intraoperative blood loss, drainage tube indwelling, ambulation, hospitalization time in the study group was less than those in the control group ( P<0.05); postoperative 1 day, 3 days incision VAS,COR and ACTH concentrations in the study group were lower than those in the control group ( P<0.05); morbidity in the study group were lower than those in the control group ( P<0.05) . Conclusion:Minimally invasive thoracoscopic mediastinal medi-astinal tumor resection has good efficacy, less invasive, and the patients have low stress, fewer complica-tions, which is worthy of clinical application.
