急性心肌梗塞冠脉溶栓后应用低分子肝素的临床观察

目的观察低分子肝素(LMWH)速避凝在急性心肌梗塞(AMI)冠脉溶栓后应用的临床价值.方法将61例AMI冠脉溶栓患者分为两组A组(28例)溶栓后给予低分子肝素(LMWH)速避凝腹壁皮下注射(0.4m1/d,连续7d);B组(33例)溶栓后给予普通肝素(UFH)腹壁皮下注射(50mg/12h,连续7d).结果B组较A组总的临床事件发生率高,统计学分析差异有显著性(P<0.01),其中梗塞后心绞痛、出血发生率B组明显高于A组.B组治疗后血小板(PLT)明显减少,凝血酶原时间(PT)明显延长,统计学处理差异有显著性(P<0.01),A组治疗前后差异无显著性.结论LMWH速避凝用于AMI冠脉溶栓后辅助治疗较UFH临床疗效好,出血并发症少,无需实验室监测.

Clinical study of effect of Iow molecular heparin in treatment of AMI undergoing thrombolytic therapy of coronary artery

Objective:To determine the therapeutic effectiveness of low molecular heparin(LMH)in acute myocardial infarction (AMI)patients undergoing thrombolytic therapy of coronary artery.Methods:61 parients with AMI undergoing thrombolytic therapy were divided into two groups(group A=28,group B=33).The patients of group A were treated with LMH(0.4 ml/12 h subcutaneously,7 d),the patients of group B were treated with unfractionated heparin(UFH)(50 mg/12 h subewtaneously,7 d).Results:Cardiac events cururred in group B was higher than that in group A( P <0.01).In group B,platelets(plt) after treated prolonged significantly reduction,prothroinbin time(PT) after treated prolonged significantly( P <0.01).Conclusions:LMH in AMI undergoing thrombolytic therapy of coronary artery was more efficient and safer.
