耳穴贴压对减缓青少年近视效果的系统评价

目的:运用系统评价的方法对耳穴贴压减缓青少年近视的有效性和安全性进行评估。为耳穴贴压治疗青少年近视的临床运用提供证据。方法:在制定严格的纳入、排除标准基础上,全面检索中国期刊全文数据库(CNKI)、万方数据库、重庆维普科技期刊数据库等数据库,并进行严格的文献筛选。在对纳入的文献采用Cochrane Handbook 5.0提供的"偏倚风险评估"工具进行严格地质量评价的基础上,利用Review Manager5.3软件进行定性和定量分析。并在95%CI(Confidence Interval)可信区间表示下,将比值比0R作为效应量选择。结果:共检索出286篇文献,纳入9篇符合标准的研究,1篇研究质量等级为C,其余8篇均为B。其中2篇为耳穴贴压与针刺治疗的比较研究,有效率:M-H OR为1.44,95%CI[0.84,2.47],P=0.180.05;3篇为耳穴贴压与眼保健操的比较研究,有效率:M-H OR为3.26,95%CI[1.81,5.88],P0.0001;4篇为耳穴贴压与眼药水、滴眼液的比较研究,有效率:M-H OR为5.18,95%CI[2.12,12.64],P=0.00030.05。安全性:偶见胶布过敏。结论:耳穴贴压减缓青少年近视优于眼药水、滴眼液,可能优于眼保健操,但尚不能得出优于针刺治疗的结论,未来仍需更多的高质量RCT研究进行论证。

Systematic Review of Auricular Pressure Therapy in Treatment of Juvenile Myopia

Objective:To assess efficacy and safety of auricular pressure slowing down the juvenile myopia.To provide evidence for Auricular acupressure treatment for juvenile myopia clinical use.Methods:Develop strict inclusion and exclusion criteria,searching comprehensively and screening literature strictly about CNKI and Wanfang data system and so on.According to the "Assess the risk of bias" provided by Cochrane Handbook 5.0,evaluate the quality of included RCT study strictly,and also do the qualitative and quantitative analysis by Review Manager5.3.The odds ratios is choose as the effect size selection,and using 95％ confidence interval representation.Results:Totally retrieved 286 documents,9 documents are included,the quality rating of one document is C,the others is B.Two documents are about the comparison between auricular pressure and acupuncture treatment,efficiency:M-H OR is 1.44,95％ CI [0.84,2.47],P =0.18 ＞ 0.05;three documents are about the comparison between auricular pressure and eye exercise,efficiency:M-H OR is 3.26,95％ CI [1.81,5.88],P ＜0.01;four documents are about the comparison between auricular pressure and eye drops,efficiency:M-H OR is 5.18,95％ CI [2.12,12.64],P =0.0003 ＜ 0.05.Security:tape allergy occasionally.Conclusion:Auricular pressure is better than eye drop,may be better than eye exercise,but can't know whether auricular pressure better than acupuncture treatment on slowing down the juvenile myopia.We need more high quality RCT study in the future.
