宫颈封闭加无创性徒手扩张推举宫颈口的产程观察

目的探讨宫颈封闭加无创性徒手扩张推举宫颈口在产程中的作用。方法将初产妇224例分为观察组及对照组各112例。观察组于宫口开大5cm时，以阿托品0．5mg加生理盐水2mL于宫颈水肿处或宫颈2、10、12点处分别进行等量注射，并施行无创性徒手扩张推举宫颈口。结果该方法可使分娩过程中宫颈口单向主动扩张与被动扩张相结合，产力增强，减少胎儿先露部下降阻力，促进胎头内旋转过程，使宫颈口扩张、胎头下降速度加快。结论宫颈封闭加无创性徒手扩张推举宫颈口能缩短产程，降低头位难产率和新生儿窒息率，提高自然分娩率及分娩质量。

Observation of birth process with cervix hermetization and noninvasive unarmed expansion of cervix opening

Objective To explore the role of cervix hermetization and noninvasive unarmed expansion of cervix opening in the observation of birth process. Methods 224 cases of primiparas were randomly divided into observation group （n= 112） and control group （n= 112）. When the cervix opening expanded to 5 cm in observation group,atropine 0.5 mg and normal sodium 2 mL were partes aequales injected at the dropsy place or 2,10,12 spot of cervix, while noninvasive unarmed expansion of cervix opening was operated. Results The method united one-way initiative and passive expansion of cervix opening, enhanced the force of labor, reduced the resistance of foetus presenting part, promoted fetal head internal rotation, resulting in cervix opening expansion and acceleration of fetal head drop. Conclusion Noninvasive unarmed expansion of cervix opening shortens birth process,decreases the head position dystocia rate and neonatal asphyxia rate,and raises spontaneous delivery rate and quality of childbearing.
